Chlamydia Azythromycin and Doxycycline failed ??

I can't really forgive myself for hooking up with someone I met online. 2 seconds of unprotected sex got me Chlamydia. anyway.. when I noticed burning while peeing, testicle discomfort, frequent urination and yellow discharge, I rushed to the doctor immediately. I was given Agumentin for 3 days which certainly helped which confirmed that i have STD not the UTI.
Later I was given single dose of 1 gm ceftriaxone and 100 mg doxycycline.  Although it gave me massive stomach upset and and caused me to have hemorroids and fissures, I completed the course. which did not give me 100% relief. I went to see the doctor again and I was on  2 gm ceftriaxone IV for three days. which certainly did not work. i still had all the symptoms.
finally Chlamydia was found out in culture test. Dr gave me 500 mg Azythromycin and 100 mg Doxycycline for 7 days. I took first 500 mg azythromycin tab in the morning at 11:00 and second one at 9:00 pm. i do not know if it was right as I read in the post that i should have been taken in one go. It has been four days already since i am on Doxycycline again but i started getting pain in my left testicle and left side of my penis. I do not see any noticeable discharge but I still have burning while peeing and frequent urination. I am so scared and paranoid.
Please help me to answer my questions below,
1. shall i take Azythromycin 1 gm again? or it is too late?
2. did my body has created resistance against Chlamydia?
3. Shall I wait to complete the course or I should go and see Urologist ?

PS: I did not have any sexual intercourse with anyone during this time.
