Epidydimitis and STD

Hey there,

Just under a month ago, I received oral sex from a sex worker for less than 2 minutes. About 10 days later I started to feel some aching in my left testicle.   I also had some symptoms which resembled a UTI. I had sex with my girlfriend within the time frame of me receiving oral and feeling the onset of symptoms I then went into a walk-in clinic where the physician diagnosed me with doxycycline for 7 days. I felt as if they were working and by the end of the 7 days felt normal. About 5 days later I felt the pain in my restocked, groin, lower abdomen and lower back return. I called my family doctor and she has now prescribed me with 10 days of levofloxacin and has requested that I go in for a urine test to check for chlamydia or gonorrhoea. I also had sex with my girlfriend about 4 days ago before I felt the symptoms return after I had finished my doxy - she has not shown symptoms of anything this entire time.

PS. I found out today that consuming calcium with doxycycline significantly reduces its effectiveness and I had a bowl of yogurt right before taking the antibiotic every day that I was on it.

My Questions:
1) how likely is it that I transmitted gonorrhea or chlamydia from my oral encounter?
2) How likely is it that I’ve passed either of these onto my girlfriend?
3) would the combination of doxycycline/levfloaxcin cure these STDs?
4) If I do a urine test within the next two days will they even show up now that I’ve taken a full round of doxycycline?
5) should I feel 100% after this round of levofloaxcin?
6) If my tests show up negative does she need to be tested?

It's hard to say what's happening because you haven't actually been tested for anything. That's a shame.

From receiving oral sex, you can get gonorrhea or chlamydia, though gonorrhea is more likely. You could also get NGU, which is an infection in the urethra.

Doxy and levfloaxcin would cure chlamydia and NGU, but not gonorrhea. Now that you've been on doxy, it may give you false negatives on chlamydia if your antibiotics failed.

My Questions:
1) how likely is it that I transmitted gonorrhea or chlamydia from my oral encounter?

Gonorrhea is more likely with oral sex than chlamydia, but both are possible.

2) How likely is it that I’ve passed either of these onto my girlfriend?

If you had either, or NGU, it's pretty likely.

3) would the combination of doxycycline/levfloaxcin cure these STDs?

Neither would cure gonorrhea.

4) If I do a urine test within the next two days will they even show up now that I’ve taken a full round of doxycycline?

Gonorrhea might. You'd probably get a false negative for chlamydia. Test anyway and see what it says.

5) should I feel 100% after this round of levofloaxcin?

Not if you had gonorrhea.

6) If my tests show up negative does she need to be tested?

Yes, because you could have gonorrhea.

You could also have something non-STD related.

1) do you believe that the first round of antibiotics failed because I drank and consumed calcium or do you believe it’s more related to the nature of the infection.

This is something you should ask a pharmacist. The alcohol probably wouldn't cause a failure, but I don't know about the calcium.

Just a little comment here.
Yes, there is some interaction between calcium and tetracycline antibiotics (which includes doxy), that is one of the reasons doxy binds with calcium in your bones and teeth.
But FDA (as well as regulators in other countries) gives no recommendation on limiting dairy foods while on doxy.
A "full bowl of yoghurt" is a good thing, it might decrease doxy absorption a little, but certainly would not lead to a failed 7-days course treatment. If it were that way, it would make doxy quite a useless drug for most cultures with a dairy-rich diet.
While I don't think you have gonorrhea, we still can't exclude it. You need to re-test and I would recommend have your regular partner tested for gonorrhea and chlamydia; there is a risk for your re-infection.

Sorry, one last question/update:

I also drank heavily 2-3 nights out of the 7 days I was on doxy.
