Mycoplasma genitalium?

Hi all,
I have been experiencing ongoing issues for a good long while now and have read comments from other members (particularly BadgerMG)  regarding Mycoplasma Genitalium.  I have been tested (by NAAT/PCR) for this infection specifically several times.  I have been careful to make sure I have not been on any antibiotics and have taken first void urine after waiting at least 5 hours (that's about as much sleep as I get) only.  My results have always been negative and yet I still get NGU like symptoms.  I know for certain that I don't have Chlamydia or Gonnorhea and have also been tested for UU & Herpes. Can I trust these results and move one?  My last unprotected sexual encounter was so brief and fleeting (grinding through underwear) that my sensible mind can't believe that my symptoms would be for an STD....yet here I am asking a question that I would answer negatively if it were posed to me.
Badger....anything you can help me with here?

How long have you been fighting this? What test are you using for MG?

There are new more accurate tests available. Your doc would have to order it. Just search FDA approved
Mycoplasma Genitalium test on google. There’s finally a FDA approved test.

Also, reach out to the UAB mycoplasma lab in Alabama, they have a really good test too.

I would test FVU and semen or ejacualate. This would sample the prostate and ejaculate. I’d send in urine and ejaculate all in one sample to maximize load.

A ray of hope...if you finally get a positive, the FDA just approve Lefamulin, which has high activity against MG in vitro. If you’ve failed Moxifloxacin, I’d push for a minocycline/Lefamulin combo. It seriously is the only bit of hope that has come along in many years.

I don't think he's in the US - I believe the UK, but I may be wrong on that. Does that change anything in your answer?

I have used several commercially available tests. The last one I used was from Randox laboratories. Large company who also do work for the police. Every time it's come up negative
I've also used tests that go to a lab called TDL in London. The Doctors Laboratory. Private STD clinics use them.

Put in for an import of Pristinamycin. I’d still pair up minocycline with it.

Why are they a better test? In no way am.i saying they're not I just really want to get to the bottom of this recurring and frustrating problem
SpeedX is the only available test that automatically checks for macrolide resistance. Plus their assays have been verified and are very accurate.

This all comes down to sample load. If you’ve had this a long time and have been on courses of antibiotics, but still have prostatitis or NGU symptoms, then you need to keep testing further.

There’s something behind this and it’s not just some random thing.

Did you for sure test for mycoplasma genitalium?

Often, doctors will order ureaplasma/mycoplasma tests and they are really testing for hominis. Which is useless. Also ureaplasma and mycoplasma hominis are known as the “genital mycoplasmas” which can cause confusion.

The test needs to be specific to mycoplasma genitalium.

Any eye issues or inflammation?

Here’s what I know.

I know of many false negatives in MG testing.

It’s like the std version of Lyme Disease.

Clinicians are testing out of protocol out of sheer ignorance and then sending off people with no answers, or worse yet, putting people on 30-60 days of cirpofloxacin and levofloxacin which has no effective cure against MG and only makes Moxifloxacin useless because MG gains resistance from cipro/Levo.

Moxifloxacin is still a hope for 70% of the cases. For many due to prior treatment of a flouroquinolone its useless.

You options are test full ejaculate and FVU together. If that comes back negative then it’s not MG.

If it’s positive, then

1) take a long 3-4 week course of Moxifloxacin
If that fails...
2)try to get on long course of minocycline. Ideally adding Pristinamycin together with minocycline would be the best option.
3) some how get Lefamulin.

Hey again.  The test you mention looks great but looking into it it's not something for home use and I wouldn't have a clue how to make sure I got tested using this test.  I can't just demand it from my doctor, it doesn't work like that here.  There certainly aren't any tests available here (for home use) that check ejaculate.  Just NAAT tests that claim to be very accurate.  The most recent one was from a very reputable company

Az & Doxy in the past.  It has been far better, non-existent really until recently.  By all means read my conversation with the very helpful AuntieJessi.  (thanks again Jessi) This most recent 'flare up' for want of a better expression comes after something that I would certainly count as low risk/no risk if I were trying to help someone else.  Perhaps my anxiety has got the better of me

Ok, well all I can say is I would test your ejaculate. You can literally put it in with your FVU. The test doesn’t care what type of sample it’s testing. It will simply be looking for the dna in the urine/ejaculate. If it’s in there, it would most likely show up because you’re maximizing the amount of bacteria. As these infections drag on, the concentration of bacteria is no longer in the urethra, it’s in the prostate and epididymis. That’s why it’s so important to get everything accurately tested.

I know people who initially tested negative multiple times, then finally test positive. Then after long antibiotic courses (2-3 months) test negative again, but prostate issue (which strongly overlap with NGU) persists. The FVU will test negative. That’s why it’s so important to include the full ejaculate.

Getting under the care of an infectious disease doc is crucial, or seeking care at a std clinics would help too. Urologists are useless when it comes to this type of stuff.

If you don't mind my asking, making the assumption you had MG yourself, what symptoms did you have?
It started with with the typical NGU symptoms but then has become more prostatitis type of issues. This has been 2 years and I still will have a cloudy discharge on a hard bowel movement every once in a while. I have dribbling on urinating and just over all discomfort in the penis and up the rectum when it flares. I’ve lost who I was prior to this nightmare and I don’t know how much longer I can take it. I’m just trying to help as many people as I can.

Grinding through underwear? Did I know it was through underwear? There's no risk at all if you have any clothing on.

Your first post said you had unprotected sex, which is quite different in terms of risk. Am I confused?
