Pain while urinating, no relief 4 days after antibiotics

I'm a 30yo straight male, highly sexually active with various different partners and one night stands.

I had two prior infections of gonorrhea before over the years, the last one being 6 months ago. 5 days ago I noticed the all too familiar slight pain when squeezing my penis and knew that this must be the very early on-set of gonorrhea symptoms.

Sure enough, the next morning the symptoms developed into painful urination. Given my lifestyle, I assumed it was gonorrhea or chlamydia. I got a urinalysis which showed mildly elevated WBC and bacteria levels. I went to a urologist and was given an immediate shot for gonorrhea and 1g (2x 500mg) azithromycin initially, with an additional 500mg to be continued each day for 3 days. Given my activity and symptoms it was pretty obvious what it was. The only thing lacking was a discharge.

After day 3, the painful urination did not subside at all. I found this very strange since last time I got the gonorrhea shot the painful urination stopped the exact same day. Also, I'd expect the azithromycin to offer at least some relief by now if it's chlamydia. This made me think that this might not be an STI at all, which is strange given the fact I've had 5 high risk sexual encounters in the last month alone with different women. I was certain it was an STI.

Last night I broke into a fever at 39C with extreme shivering/chills. Went back to the urologist the next day. Even though until now I've had no discharge, there was a slight clear discharge on the top of my penis during the consultation. This was unlike a typical cloudy discharge; it looked more like clear urine. It was sent to the lab for a sample analysis, and came back with no bacteria found, however was very high in "pus" cells. The urologist has extended my azithromycin for a further 3 days (500mg each day).

Now I'm puzzled about this since all my reading on the Internet shows that azithromycin is normally taken as a single 1g dosage, and not for a whole week. What would be the reason do you think for continuing azithromycin?

Lastly, I masturbated as a 'test' to see if it was painful. I experienced a very painful orgasm for the first time in my life. I wouldn't say the painful urination is worsening, but it's not getting better either. It's now day 4 and still no relief. What should I do? Just continue the azithromycin for now? What could be causing this as surely by now I should be having some relief?

Hi it would be best to talk to your doctor about this but with urethra infections like gono there would also be a white puss discharge and burning while urinating, if its just pain it could be an injury inside from the infection also when men get gono a few times it can cause the uretha to thicken from scar tissue causing pain as urine is now being forced out the tube. Just a thought.
