Protected sex with African hooker

I recently  got drunk and went to a bar where I live in Oman. There were some hookers at the bar. I took one home with me. We had protected sex for about ten seconds. I was semi hard so I gave up having sex with her. The condom remained on an intact throughout. However, she then proceeded to give me a rimjob. I also fingered her and then masturbated.

She told me after that her boss beats her and she had a burn scar on her face which was from her boss. I am new to this country and it’s awful that this happens. She seemed so happy at the bar.

My main concerns are should I be worried about:

1. Protected vaginal sex - risk of STD
2. Unprotected rimjob. She spat in my anus a few times as well as licking it for a few minutes.
3. My masturbation of myself with fingers that may have had vaginal fluid on them.
4. What happens if I rubbed my eye by mistake with her vaginal fluid?

I have booked into a private clinic in London on day 21 post exposure (this Friday). They run the duo test.  I will also test for syphilis, gonorrhoea, clamidia, and hepatitis A. Will that be conclusive?

The HIV forum said I was risk free for HIV but I wanted to check in case of risk for others too.

I have seen conflicting statements by doctors. HHH has said its very low risk if receiving anything from the rimjob. However  the following link the doctor says there’s a 40pc risk per episode.... that’s freaking me out.

The rates of syphilis are low generally, and oral sex of any kind is lower risk than vaginal or anal intercourse, so when HHH says receiving a rimjob is very low risk, he is taking all of that into consideration.

The 25-40% chance of getting syphilis is if the person giving you a rimjob has syphilis. These are two different scenarios. HHH is giving a general risk. Dr. Cummings is giving the chances if someone is infected.

"If she has syphilis then you run a risk of between 25 and 40% per episode of acquiring syphilis from her." Note the "If she has syphilis...". She would also have to have an active syphilis sore in her mouth in order to transmit it to you via a rimjob.

Low, but there is still a risk. Using a condom provides protection for gonorrhea, chlamydia and NGU. It offers some protection against herpes, syphilis and HPV.

Probably nothing. STDs die pretty quickly after being exposed to air, and hands aren't a good way to transmit STDs.

You could also get genital herpes type 1 from the rimjob. If you already have hsv1, you can't get it again.

You don't need most of the testing you are getting done. Ask for a gonorrhea test anally, a syphilis blood test, and a type specific IgG herpes blood test (and don't be surprised if they won't do that. The UK doesn't like herpes blood tests at all. You probably won't be able to talk them into it, either, but give it a try.)

If you have other possible exposures, then get all the testing they offer. If you are only concerned with this exposure, you don't need HIV, chlamydia or NSU. If she performed oral sex on you (her mouth to your penis), then ask for NSU.

Thanks v much aunt Jessie for that helpful advice. My exposure was just that once. It will be 21 days on Friday (when I take my test) since the exposure. Is that enough time for the tests that you recommend to be conclusive?.

Given that she is a hooker and not in a western country where they look after themselves (she is beaten by her pimp she told me after, to my horror) I can only assume she is infected with all of these horrible diseases/infections. I didn’t notice any sores on her mouth. Overall, would you so I’m at low risk from catching an std?

Sorry for the stream of consciousness - what’s the risk of gonorrhoea from the rimjob? Or any  std other than syohilis? Assuming that’s low?  
Surely I would have got some symptoms by now if I had been infected? Still I guess it’s still worth getting the recommended tests done as you said, right?
I'm sorry you were so shocked by her story, but it's a very common one, around the world, even in developed countries. Many sex workers are treated horribly, both by their pimps and their customers.

She used a condom with you for intercourse. That means she is taking care of herself. She is protecting herself from you and her other clients far more so than she is protecting you.

Even if we assume she has all the STDs (which she really probably doesn't), it's still low risk for you. The mechanisms of oral sex and rimjobs make it less efficient in transmitting STDs.

The UK doesn't like herpes testing because they think that there are false positives (there are), and that it's easier to determine if someone has herpes by swabbing sores (true). There are downsides to this - not everyone gets symptoms, meaning that without blood testing, people don't know they have it, and are therefore unknowingly spreading it being the biggest one.

Yes, the risk of gonorrhea and syphilis from a rimjob is low. By now, you'd probably have symptoms of gonorrhea if you had it, if you got symptoms. Not everyone does. The average time to get a syphilis sore is 21 days, but can go to 90 days.

If going to London is a huge imposition for you, financially or otherwise, I wouldn't bother. If you can go easily, there's no harm in testing. Your syphilis test won't be conclusive until 3 months, so if you can only go once, wait.
Thanks for the comprehensive message. In fact, I gave her the condom, she didn’t have one. This raises the risks of her having an STI as she most likely exhibited the same behaviour with others who Wrecklessly have unprotected sex. I happen to be heading to London anyway, so it’s not an imposition. However I will be seeing my girlfriend who will find it unusual if I use protection with her (she’s on the pill).

Will she be at risk ? I don’t want to harm her obviously. Given this was a mistake I’m never doing again I don’t think it’s a good idea to tell her. I want to put this behind me.
Also, In calculating the risk of syphilis I should multiply 40pc with the risk of her actually having an open sore or chancre. What proportion do you think of syphilis carriers would have an open sore that could cause the spread? I’m not sure how common syphilis is in general either, but I’ll assume it’s 100pc for these purposes considering the source
Well, regardless of her behavior with others, you used a condom. That helps.

Syphilis is only infectious when someone has symptoms. If she didn't have an oral sore, she can't give it to you via the rimjob.

I'm having a hard time finding any stats on syphilis rates in Oman, but don't assume it's 100pc. You might be able to google it and find more since you're actually there. Syphilis rates in every country are far lower than that of chlamydia, gonorrhea and HIV. If you don't have symptoms, she won't be at risk. Will you be seeing her before you go to the clinic? Ask the clinic to do an exam and see if they see any unusual symptoms that might indicate syphilis, but I'd be really surprised if you got syphilis from this.

I can't give you a proportion. If she had syphilis, it would depend on where she is in the disease process if she had a sore or not.

Thanks so much for another thoughtful answer. The first thing I do when I land in the UK tomorrow is get a test. The results should be same day as they offer that service and I’m paying lots of £ for it! That’s unless there is a strike by the courier service  which is used to take the samples to the lab  which I heard there is... anyways.

I don’t see my gf until 3 days after I land (3 days after my test). If the results aren’t ready by then I won’t get intimate with her just to be safe. At least those I’m at a small risk of getting are curable (apart from herpes) and I shouldn’t worry about HIV as you mentioned. This has reassured me massively. You are doing an amazing job doing this - I wish you were my doctor!

I took a test for All the things we mentioned including HIV and all cane out negative after 21 days following the event. The Doc said that the results are fully conclusive and the window period and passed and I can now relax . Thank you for your help and advice. All the best. R
