Questions regarding unprotected oral encounter

Background:  Got stupid drunk with some friends, met up with a CSW at a bar, and wound up getting unprotected oral (stupid).  Feel really bad about it the next day.

2 days later start feeling a pinch in my penis tip and I have to urinate frequently.

No common symptoms from this enoucnter:  no discharge, no spots, no pain in urinating, no itchiness on the skin.  The only symptom I have is a persistent tingly pinch at the tip that won't go away - but it's not bad enough to cause me to lose sleep.  It feels like there is a slight blockage as my urine does not start with much gusto, it builds up to normal pressure, then it kind of sputters out at the end.  There is no sensitivity to touch, just a mildly annoying feeling at the tip.

Anyways, so now its 7-8 days later and I go in to see the doc, prepared with some knowledge I've learned here.
I purchased a full urinalysis screening that includes: Chlamydia, Gonorrhoea, Monilia, Trichomonas, Mycoplasma/Ureaplasma, Urine microscopy and culture

Since it was a short encounter, I know that the likelihood of anything is small.  And the likelihood of syphillis or herpes II from oral is also very small.  Plus those tests were fairly more expensive, and I just don't have that money at this time.

I am scared of spreading anything to my partner (again, made a stupid drunk mistake and don't want to harm her)

Anyways; the doc gave me a shot of 500mg Ceftriaxone and 6 x 250mg Zithromax.

My Questions:
1. What is the proper way to take the Zithromax?  The nurse mumbled through the instructions, and then I read online here: http://bacteria.emedtv.com/azithromycin/azithromycin-dosage.html that for STDs you are supposed to take it all in one dose.  Should I spread it out like I think the nurse was telling me or take 1g now; 500mg tomorrow; or take 1.5g all at once?

2. I get my results back in 3-5 days.  How long does is take for these types of antibiotics to render the bacteria inert?  My girl is coming to see me next Wednesday.  9 Days should be enough time right?

3. If the "pinch" doesn't go away; any ideas on what else it might be?  Some other UTI?  A fungus?  A prostate infection?

Lastly, I want to say thanks to Grace and Vance in advance (no pun) for all their work in easing the minds of all of us knuckleheads.  I know you get asked the same questions over and over and sometimes are never thanked for your input.  It is appreciated.

Bacteria - Nil

Others - Nil

Microbial Examination:
Epithelial Cells - Occasional
Pus cells - nil
No yeast cells, Trichomonas vaginalis, or gram negative intracellular diplococci or clue cells seen.

NSU Culture (Mycoplasma/Ureplasma):
Mycoplasma culture - No Growth

GC Culture: No neisseria gonorrhoeae isolated

Talked to the doc; told him that the Canesten (1% Clotrimazole, 1% hydrocortisone) I was using was helping a ton to get rid of the sting, tingle, pinch.  I told him that when I masturbated it hurt immediately after.  That if I drank a coffee or coca cola it hurt within 5 minutes.

He concluded that I probably got a slight abrasion due to the oral and that the coffee/coke stimulant was causing an inflammation in my urethra around the abrasion.  When the abrasian swoll, that's why i was feeling a pinch and why my urine stream seemed less powerful (a dribble at times).

With the masturbation, he said because semen has a slightly different pH level than normal in the urethra, that it could irritate a small abrasion.

This all seems plausible to me, so I am satisfied.  In fact, ever since I got my results, I have felt pretty normal.

I will continue to apply the cortisone for another week or so until I feel fully healed, then test myself out with a Starbucks to see where I am at.

I will report back next Friday if possible.

On follow on question:

It's still sort of tingly, but getting better (i think).

I recently masturbated and the sting increased quite a bit upon climax.  Then the tip was red around the urethra opening...

Is that any kind of symptom or sign?  I am still waiting on my results.

Yeah, so I read Grace's reply in another thread; and she said it could be a fungus due to the symptoms...

I did notice this morning that the "pinch" had gone away for the most part to where I was barely aware of it... so I drank a cup of coffee or 2 at about 9:00am and the pinch almost immediately came back.  Does caffeine feed a fungal or yeast reaction?

1.  Thanks, I just read the stinking box... take 4 tablets STAT (1g)

So I guess I'll take 1g today, and 500mg tomorrow.

1. We can't give that type of medication advice, follow up with your Dr.

2. 7 days is the recommended waiting period
