Red scrotum, itching and frequent urination. Tests negative. What to do next?

Hi everyone,
I have been struggling for years with what I am pretty sure is a STD, but I can’t get a diagnosis or much help from the doctors I have seen so far. It all started a couple of years ago when I had protected sex with a stranger. After a couple of hours I immediately noticed redness on my scrotum, moist skin and a weird feeling that is something between itching and burning. About a week later I began to have tingling in my legs and bumps on my anus that were diagnosed as haemorrhoids (although I am not sure that was a correct diagnosis). A week later I did a blood test for STDs (and repeated in a month and three months). The only positive results at the time were Chlamydia Igg and HSV-2 IgG, while both IgM were negative. My doctor assured me I am fine, my symptoms slowly started to fade, but were still present (reoccurring in intervals of a couple of weeks). A few months after I felt better, convinced myself I was fine and I started a new relationship. Right after I started having sex again the scrotum redness and itching feeling came back. The only other new symptom I had is a frequent need to urinate, but no pain and no discharge. My new girlfriend started having similar symptoms, but for her it got much worse with time. It started the same with a weird feeling of itching or burning, but progressed to move severe symptoms including white/yellow discharge, pain during sex, small bleeding after sex, cessation of menstruation and gastrointestinal issues. We both got tested multiple times for STDs including HPV, Chlamydia, Gonorrhoea, Trichomonas, Mycoplasma and Ureaplasma from both urine and swab samples, but all results came back negative (PCR method). Her gynaecologist diagnosed a PID and a closed fallopian tube, so we both got treated with Azithromycin. That seemed to help and her condition improved. It was good for around month and then again all the symptoms came back for both of us. I am really frustrated and out of ideas what this is and what I can do about it. I have seen a lot of doctors and most of them did not take me seriously as all my test results came back negative.

Regarding skin swab? Yes, a dermatologist did, but she ensures me its not a fungal infection. She says that looks and behaves differently.

My HSV2 IgG was positive after 2 weeks and 1 month. I did a test again after a year it came back negative. HSV2 was my first suspicion as well.

Chlamydia IgG was also positive, but it was negative on repeated DNA swab and urin test so I think we ruled that one out.

I am pretty sure this must be sectually transmited as sypmtoms fade when avoiding sex for some time and reaprear (like in a never ending circle) when having sex.

Hsv2 isn't my suspicion - I was just trying to confirm that your first test was a false positive, as those happen often.

I don't know that it's sexually transmitted. STDs don't just fade, and then reappear with sexual activity. If you have an STD with symptoms, those symptoms stay until you're treated (with the exception of herpes, which you don't have).

That's why I was thinking allergy. That maybe you are using something when you're having sex - either condoms, lubes, something to clean up with, etc., and then those symptoms come back and then fade.

If it's not an allergic reaction, then maybe it's a chemical that's harsh that both you and your gf are reacting to.
Thanks for your input. I also considered condom allergies, but that would not explain the frequent urination that goes with the redness and itching. It would also not explain why my GF is having STD-like symptoms. I was recently diagnosed with chronic prostatis and I thought all of this must be connected somehow and have a common cause. Do you think it could be caused by a bacteria that is not an STD?

Have they sent your urine for culture? Done any swabbing to see what bacteria is in your urethra? Done the same for your girlfriend, though her swabbing would be vaginal. I know they've done STD testing, but have they just looked for bacteria?

Has anyone done a swab of your skin, or a biopsy? Has anyone mentioned a fungal infection? Atopic dermatitis? Eczema?

You could easily transmit a fungal infection to your girlfriend. You wouldn't transmit eczema, though.

There is no STD that would give you symptoms within a couple of hours. You might see symptoms of an allergy, though, that quickly. Have you tried benedryl or hydrocortisone cream?

What was your result on your hsv2 IgG?

Did you test positive for chlamydia with a blood test? That's not a terribly reliable method of testing.

Think of every thing that touches the area for you, and for your girlfriend. Are you using a harsh soap? A scented laundry detergent? Scented or printed toilet paper? Taking bubble baths? Are you using condoms with your gf? Maybe you're both reacting to the type of condom or a lube on it.

Other than a fungal infection or something you both are reacting to, I can't imagine what else it is because you've both been tested for everything.
