Tingle on top of penis

So back in Jan I had this weird feeling in testicles and tip of penis for a weekend, got tested for Chlamydia and Gon both cam back negative, once I found out that out that weird feeling went away. Jumping to April the testicle ache came back for about a month and a half so I got tested for STD chlamydia gonahrea syphillis hsv 1&2 hiv. All came back neg excepts hsv 1 and when I found that out a tingle/spasms feeling started happening on tip of penis. It’s been 2 months and the tingle/spasms feeling has not gone away. There is no discharge no burning when I pee, just a little redness around meatus. I went to urologist they said I had a yeast infection so they gave me since nystatin w/ triamcinolone but didn’t help. Recently I noticed I have been having lower back pain with testicle ache with the tingle on head of penis and honestly has gotten me depressed with a bunch of anxiety, also I’ve notice it’s hard for me to get erection or hold an erection don’t know if that’s related for just because my anxiety has gotten worse with these problems. If anyone can help out of calm my mind about this I keep reading that it could be trich but I haven’t had any actual symptoms of trich and I think that’s making things worse since I never got tested for it and makes me think I have it but like I said never had any symptoms of it

You haven't mentioned anything about any possible exposures, so it's hard to assess your risks for those, but does your doctor know about the erection difficulties? This sounds like a prostate issue, which you can get at any age.

I'd follow up with your urologist.

I’ve been having unprotected sex with the same girl since Jan and no I have an appointment with the urologist this Thursday which I’m going to mention
Do you think your partner has STDs? You've tested negative for everything, so it's not an STD, and if you've both been monogamous, there's no risk here.
I asked her and from what she told me she says she doesn’t have anything
Testing is the only way to know for sure. You've tested negative, though, so that's not what is causing your symptoms. Let us know what the urologist says.
If you're worried about trich, ask for a test. Good luck!
