What could it be?

About 1 month ago I had protected one night stand sex with woman & both received and gave her unprotected oral sex.

Later in the same week, performed protected anal. And I received unprotected oral.

The next week I experienced pain in my urethra area (penis & below stomach)

I went to a sexual health clinic to get tested 8 days after last exposure - which came back negative (for
Chlamydia, Gonorrhoea, hiv & syphilis CIA urine sample, penile swab & blood)

I went to the doctors the day after and they gave me azithromycin for suspected  Chlamydia Or gonorrhoea. Please note that I had no discharge. The dose was 2 x 500mg on day 1, 500mg on day 2 and 500mg on day 3

The symptoms reduced but not completely. The doctor also took a ruined sample which was ok.

I went to the sexual health clinic 2 weeks exactly after my last exposure. I took tests for Chlamydia, gonorrhoea (penile swab, urine sample) . The results were negative.

I took a home test exactly 2 weeks & 2 days after exposure and the results were also negative (for all mentioned including hep B)

I went to the doctor for a urine sample to test for an NGU or UTI. Their initial dip stick result is negative. They will however send my urine same to the lab for a full result.

The reason I have done so much testing is because I am worried about spreading something to a partner I am seeing. Yes I know I messed up.

Also I still feel some little discomfort even after azithromycin for 1 week & 5 days after my last dose.

Should I be concerned?

I would not worry about gonorrhea or chlamydia. Azithromycin would have cleared out chlamydia and gonorrhea would be detected by testing.
Your symptoms may be caused by some other bacteria or virus or just by anxiety or dehydration.
HIV and Hep B are very low risk from oral sex, there are no documented cases of Hep B transmission via oral sex.
The only things that could be missed are syphilis and herpes. I do not say you should worry about those, as the risk is not too high.
You may test for syphilis at 6 weeks, if you find any symptoms, better check them with a doctor.

Thank you for your answer. For some reason I still feel serious pain. It’s got worse today. I think it could be some other bacteria which doesn’t help because I have no diagnosis for the pain I feel.
If you experience serious pain, you should get it checked with a doctor. Do not think STI, let the doctor do the diagnosis.
Find a urologist, get your prostate and testicles checked.
STIs other than gonorrhea are never associated with strong pain, and you don't have gonorrhea.
