Small rash on penis head after 20 days of intercourse

Age - 22 yrs , male.
I have had sex 58 days ago. I have used condom. While removing the condom, my 2 fingers touched the tip part of condom which had gone inside vagina. Immediately after it I went to wash my hand with soap and water. Then I washed my penis with soap and water using the same hand. I had small tear in my penis.

After 2 hours. I wore another lubricated condom( didn't have sex this time ) and the lubricant touched the torn part of my penis. I felt my penis become numb. I removed the condom and washed my penis.

After 6 days I had pharyngitis and doctor gave 10 tablets of amoxycillin to take for 5 days ( from day 8 to day 13).

On day 18, I noticed a small  dark blueish patch on head of my penis( no itching, no swelling). On same day afterward a one year old accidentally stepped on my penis head when I was holding him on my arm.

On day 21 I showed It to doctor ( didn't say that I had sex). He suspected  fungal infection and gave me antifungal cream and 3 tablets to take one every 7 days. I used the cream twice  a day on penis and the patch disappeard within day 25. But there remained some burning sensation while urinating.Same doctor recommended to drink more water and gave urine alkanizer which worked.

Now , after 50 days , I am starting to feel mild pain on my testes, penis head  for only some time  every day. But the burning sensation remain same( neglegible when I drink water). I went to another doctor , told him about everything about sex and pain. On day 55 He tested me  for HIV type 1 and 2, TPHA, VDRL,  HBsAg Rapid ... and all came negative. He gave me 5 tablets of antibiotic(azithromicin) for the throat discomfort.. and told me that I am having venerophobia. And It's true... I am afraid. But there may be some STD . I have taken 3 tablets of azithromicin already and the pain and everything became a bit better but pain and burning sensation  still happens sometimes. And happens when I masterbuate.
So what is this?

"While removing the condom, my 2 fingers touched the tip part of condom which had gone inside vagina." - this is not a risk. Even if she had an STD, the exposure to air would have degraded it enough to make it not able to transmit.

If you didn't give someone oral sex, your sore throat isn't caused by an STD. It could be covid, a cold, allergies, one of many upper respiratory virus, dry air, etc.

No STD causes a blue spot or a rough patch on the penis.

Continue to drink water, and avoid alcohol and caffeine. You may just be dehydrated.

If you've had unprotected sex, or received unprotected sex, get tested for gonorrhea and chlamydia 3 weeks after you've stopped taking all antibiotics.

No not any oral sex, nothing except vaginal sex with condom.. and It was not torn. But she had touched my penis with her hand.

Yes, you mentioned that. I explained why that isn't a risk.
