Urethritis, driving me crazy!!

Hello, I am a 28m who is married. I have been experiencing numerous issues....pain during urination, soreness just below the head on one side of penis, pinching pain after ejaculation (sometimes after peeing), frequently urinating and dripping after I pee, abdominal pain, tingling on right side of body all the way up to my head, testicle discomfort, not really any discharge however my discharge has change when I am sexually aroused a lot more clear watery discharge happens then before and my semen is different no odor but just different then it was, I have tested negative for HIV, chlamydia, gonorrhea, herpes 1-2, syphilis at least 5 times, mycoplasma and ureaplasma species 2 times by culture, trichomoniasis once, hepatitis a-b-c once. All NEGATIVE, and this has been over the time span of about a year. I have been diagnosed with urethritis with no clear cause. I have also got a cystoscopy, colonoscopy, and ultrasound of testicles & abdomen. The only abnormal finding was a cyst on testical and everything else was normal. I have had treatments of doxycycline (30 days), metronidazole (10 days), levofloxacin (7 days), and most recently azithromycin (for pneumonia that they couldn’t detect, 6 days).
It gets semi-better initially with the antibiotics, but always comes back. I’m still having issues, and I honestly feel like I’m going crazy. The doctors think I’m nuts, and they don’t know what else to do. I’m beginning to feel the same way...it’s driving me nuts, and I have no clue how to get help and find a solution to this. I’m hoping getting my story out, leads me to some answers.....any help would be deeply appreciated.

It seems they have fully cleared you for STD's which is good.  Urethritis can be chronic if it goes past 6 weeks which yours obviously has.  You've tried many antibiotics, I'd try again. Are you super stressed?  For some reason this can be made worse by stress. There's also something called urethral syndrome.  https://www.healthline.com/health/urethritis-chronic#causes  

Causation seems important here.  Is there anything you can change that would impact this? Chemical irritation is a big one.

I"m sure this is very frustrating but I'd keep working with your doctor.  Hopefully it gets sorted out.

I am glad I pretty much have the clear on STDS, however my only concern is the antibiotic resistant Mycoplasma species.
I feel like that’s the only thing it could be at this point.
I am under extreme stress right now, and that’s when these problems started. I feel like stress was the trigger.
I have looked into narrowing of the tube, and results were normal.
Appreciate your response, and I will definitely keep working at it until I find a solution.
Has anyone checked your prostate? I'd say it's probably okay, given that you had a cystoscopy, but I just want to make sure they've checked it.

One thing that seems to help a lot of men in this situation is to give up caffeine entirely. (You should check out our Men's Health forum - https://www.medhelp.org/forums/show/93 - we have others with similar symptoms.) If you drink alcohol, you should avoid that, too.

Even if the mycoplasmas are resistant, you can test for them. There is a new FDA approved test - https://www.fda.gov/news-events/press-announcements/fda-permits-marketing-first-test-aid-diagnosis-sexually-transmitted-infection-known-mycoplasma I don't know where you are, but testing is easier for it now.

Doc did a quick prostate check (finger inside), nothing extensive though. May have too dive deeper on this one.
I gave up caffeine, alcohol, Everything but water for about a month and still had issues. Since then I’ve started again in moderation.
I tested twice by culture for mycoplasma and ureaplasma. I’m wanting to test by APTIMA PCR, but the doctors have been really hesitant to test for mycoplasma, they just say they have no clue about the problem and wouldn’t be able to give the recommendation because they don’t know anything about it.
Kaiser Permanente were the only ones to do it by culture. However, now I have PPO which is through independent practices, and I’ve been hitting roadblocks on finding an adequate doctor with knowledge on the issue.
Might just go to USC OR UCLA (SoCal) medical and see if the can help.

This is a little concerning?  Are you seeing STD specialists or general practitioners?   See someone who DOES have a clue about the problem and will administer a PCR test as Mycoplasma Genitalium is known for being very hard to culture.  I'm not suggesting this is what you have, if you have any infection at all, but without a PCR test you can't rule it.
I have been tested for this (not approved tests, we don't have any in the UK, but I still trust what I've had)  several times and it has always been negative but believe me I know from first hand experience how frustrating your experience is.  Find someone who will give you a PCR test and then at least then you will know one way or the other.  If it comes back negative then don't discount emotional stress
