What could this be?

Been to numerous doctors and one urologists and came up with nothing.
Started within 12hrs of unprotected sex about 39 days ago.
To make it simple, started with burning/itching in urethra and since then had some clear discharge (could've been precum this was within the 1st and 2nd day thats it), lower back pain right side (maybe stress), testicular pain in the right side (sometimes come with sitting), pain with bowel movements (prostate), frequent urination, burning usually comes after masturbation now and sometimes when sitting
Tested for chlamydia and gonorrhea both negative. Numerous urine samples and nothing. Put on Azithromycin two weeks ago for a 5 day dose in case of inflammation.
Visited urologist on monday and he found my prostate to be enlarged. Also had a scan on kidneys and abdomen, just in case something was wrong from a snowboarding injury, looked good. Also had a ultrasound of testicles and that was normal. Put on Cipro for 10 day dose. Went back today and had a cytoscope of the urethra and everything looked absolutely normal and in clean healthy shape according to my doctor. He said he saw no signs of inflammation at all or lesions. I asked about herpes and trichinosis and he said I don't not have either and would have seen signs in the urethra if I did. The burning as of yesterday was still there after ejaculation.
1. I was wondering if this could be trichonamis and should try and get metronidazole as a preventive. I know it can be hard to test for in men, just to make sure I am covered.
2. Does this sound like herpes? I know since its lasted for over a month that is not normal for the primary outbreak or any other kind of second outbreak. Also the lack of lesions inside the urethra and outside the skin anywhere is another thing leading me to believe it is not. Also have read that herpetic urethra would be very very painful which this has not been and urologist assured me it was not and just to finish the cipro and forget about the whole thing.

Welcome to our Forum.  I cannot tell you what this is but I can assure you that it is not an STD.  Let me tell you why.

There are no STDs which have the onset of symptoms within 12 hours of exposure- none.  Second, you have been tested and the tests were negative.  Third, you have been treated with antibiotics which cure virtually all  bacterial STDs (both the azithromycin and the cipro).  Neither trichamonas nor herpes should be a concern.

Sometimes after an exposure that, in retrospect, one wishes they had not had, persons tend to examine themselves and be far more attuned to genital sensations than in periods when they are not concerned.  This in turn leads to noticing what turn out to be normal sensations that might have been not noticed or ignored at other times.  This includes detection of normal urethral secretions which can be mistaken as a discharge- most men have small amounts of normal urethral discharge with is typically not noticed unless the person is looking closely as I suspect you have been.  I suspect this is a contributor to your situation.

It’s time for you to stop worrying and stop seeking medical care. If you continue, you will continue to get medications and tests from some doctors because that's often easier than convincing someone who is anxious about a sexual exposure that they don't need treatment.  EWH
