I have had this issue for 16 years, I had sleep studies, brain scans and taken many types of medication and nothing is helping.

Right about the dreaming. We usually awaken spontaneously after the last completed REM stage of the night, so that's normal. The unrefreshing sleep is best addressed by a healthcare professional.
