What do these MRI results mean?

Findings:
Cervical vertebral bodies demonstrate stable height and alignment with mild anterolisthesis of c4 on c5 and focal reversal of the normal cervical lordosis centered at c5.
Marrow signal of the cervical vertebral bodies is mildly heterogeneous. No focal lesions are seen in the cervical cord or in the included portions of the brainstem and cerebellum.
There is signal loss and loss of height of multiple cervical intervertebral discs compatible with DDD.

Findings by levels are as follows:
C2-3) Unremarkable
C3-4) No central canal stenosis. Disc bulge and uncovertebral spurring result in moderate right and mild left foraminal narrowing.
C4-5) Anterolisthesis with mild disc uncovering, but no central canal stenosis. Uncovertebral spurring results in severe right and mild left foraminal narrowing.
C5-6) Circumferential disc bulge indents the ventral thecal sac and ventral cord without abnormal cord signal or central canal stenosis. Disc and uncoverterebral spurring result in mild bilateral foraminal narrowing.
C6-7) Circumferential disc bulge indents the ventral thecal sac without central canal stenosis. Disc and a digital spurring results in mild bilateral foraminal narrowing.
C7-T1) Unremarkable

Virtual myelography is unremarkable apart from central canal stenosis as above.
Visualized paraspinal soft tissues and neck are within normal limits.

Impression:
Moderate cervical spondylosis, most significant at c4-5 and c5-6 with severe right foraminal narrowing at c4-5, but no acute abnormality.

I have trouble moving my head without pain, especially to the right. In addition, the severe pain in my neck radiates down my right arm as well as to my right shoulder blade.
A recent EMG was performed and the results indicated I have moderate Carpal Tunnel in addition to the cervical radiculopathy. My surgeon told me this is called a double hit.

I look forward to any guidance and wisdom shared as to what my options are (physical therapy or cortisone shot, or surgery). If surgery is necessary, what types would be most conducive to my healing?
