Brooke, There are so many things that can be going on. We aren't going to get this one figured out online. But severe pain can be from muscles, TMJ (if you've had arthrocentesis that should have helped), neuropathic, neurovascular, nerve impingement/entrapment, and much more. You will need an orofacial/craniofacial pain specialist to make a differential diagnosis (sorry about the technical terms, don't know what else to call it). This should involve a careful review of your symptom history, location, duration, frequency of pain, nature/quality of the pain. Should be a cranial nerve exam, physical/palpation exam. Sounds like you've had enough imaging studies already, but the Dr will want to review. Possibly diagnostic local anesthetic injections. Doesn't sound like an easy one to figure out.
