Uptake scan

My wife just had an Ultrasound and was diagnosed with a enlarged nodule.  Over the last 12 years she has been taking Levoxyl. Does an Uptake Scan determine whether she has Thyroid Cancer.

She doe not need a nuclear scan.  A scan is not indicated in the evaluation of a nodule in a patient on levoxyl for hypothyroidism.
She needs FNA biopsy of the nodule seen on ultrasound -- 90-95% of these are benign.

I have been taking Levoxyl for 14 years.  My enlarged nodule has grown to 4 cm, according to an Ultrasound. A biopsy shows that I have a smaller nodule on the isthmus that was benign and a follicular  on the enlarged nodule, which may or may not be benign.
Why are they not able to determine malignancy from a biopsy?  Should  I have surgery  on half of my thyroid or should the entire thyroid be removed.?
