Your results does show you might possibly have hyperparathyroidism as your PTH value is inappropriate for the given serum calcium value. It can be mild hyperparathyroidism. Also you can rule out all other reasons for hypercalcemia like kidney problems by doing serum creatinine, thyroid problems by doing Free T4, Free T3 and TSH (hyperthyroidism can mildly raise calcium values).
Have you been diagnosed with hyperparathyroidism? I don't think that's your problem, since with hyperparathyroidism, both calcium and PTH should be high and your PTH is in the bottom half of its range.
You should talk to your doctor about other conditions that can cause high calcium levels. Your level is actually, still within the range, so your doctor may not be alarmed, though it's definitely something to check out.

Your vitamin D level is too low, in spite of taking 1500 IU/day; It should be, at least, 50. You could bump up your dose to around 2000 or so, if your doctor agrees and retest in 2-3 months and see if your level is coming up. When it reaches 50-60, you can drop back to a maintenance dose... Vitamin D deficiency can cause quite a few symptoms, also.
