goiter with normal tsh

I had blood test done in october my tsh is 0.44 my free t4 is 1.09 my free t3 is3.0 my tpoab is 10. All these test are normal. Yet I went to my ENT and he noticed I have a goiter bigger on right side then left. Thyroid problems run deep in my family. Both my mother and daughter have been diagnosed with hashimoto's. I however am having symptoms of hyperthyroid. weight loss, dry thinning hair, anxiety, muscle spasm's like crazy, insomnia. Doctor say's all test are normal so I have nothing to worry about. I am concerned because In 2009 I had a cyst removed from my throat and precancerous cells CO2 lasered off my vocal cords. I went to ENT because I have started choking when eating.

went on the 19th of dec. my weight went up one pound and tsh level was 1.4. went back to doctor on the 22nd and weight was down to 145.4. Go for ultrasound on tuesday. Dr. said she doubts thyroid is hyperactive or I wouldn't have gained a pound on the 19th. Had to fight with her to order ultrasound. She never even touched my neck to see if thyroid was enlarged. I do know that my vit. d, 25-hydroxy is 20.2 ng/ml and I'm having severe muscle spasms in right shoulder and neck could this be the cause of all this even my thyroid being enlarged?

Considering your history you may need to have thyroid ultrasound, vocal videostroboscopy , and maybe CTscan
