Results of testing

Hi,  I recently had a ultrasound of my thyroid due to my Rheumatologist finding an enlarged thyroid by physical exam.  I have had quite a few symptoms/ health problems in the past two years. The ultrasound came back that I had a multinodular goiter with below findings.

Findings:  The right lobe measures 5.1 x 2 x 2 cm and the Left lobe 4.9 x 1.8 x 1.9 cm.
There are several small, hypoechoic nodules within the left lobe of the thyroid, the largest measuring 4 x 6 x 6 mm in diameter.
A cyst with internal septation and debris is seen within the lower pole of the right lobe of the thyroid. This measures 1.6 x 1.3 x 1.2 cm.  A 2.5 mm diameter cyst is also seen within the lower pole of the right lobe.
Normal flow is seen within both lobes with color Doppler.

My questions are how enlarged in my thyroid a little or alot?  Do I have a risk of having Thyroid cancer?  In two years I have managed to put on 49lbs even though I exercised daily and watched my diet.  I have tired very hard to lose weight and simply can't. My Rheumatologist says I have Hyperthyroidism symptoms though. Can you still have all the weight gain and still have hyperthyroidism?  It seems I have had a combination of both Hyper and Hypo symptoms.  For example: I have streaks of wanting to sleep all the time/ no energy and times of not being able to sleep even though I felt exhausted. Should I see a specialist?   I don't see my Rheumatologist until Febuary which is when we will talk about these findings!  Would like some answers sooner!!

I agree with (utahmomma) that a endocrinologist is a specialist on THYROID PROBLEMS and by the way if you see a endo and he orders a FNA which is a needle biopsy it is not that bad!  I had a TT on 12/21/07 due to Papillary carcinoma of the right thyroid and today I found out the Surgical report showed Papillary carcinoma in the RIGHT Thyroid and in the LEFT thyroid I had Follicular - variant  Papillary carcinoma and two out of three lymph nodes were positive for Papillary carcinoma!  My TSH, T3, T4 have always been normal all my life so not take any chances with your HEALTH!!

I'm no expert of size of thyroids but a couple of things.  It's hard to say what your chances of thyroid cancer are since 90-95% of thyroid nodules are benign.  However there is an entire forum of people here who never, ever thought we could have thyroid cancer.  Yet - kaboom!

The cyst with septiation (?) and debris should be watched closely and, perhaps biopsied.  It is large enough for a fine needle aspiration (FNA).  A thyroid scan wouldn't be out of the question either to determine functionality of the cyst.  

An endocrinologist is the thyroid specialist.  Since your Rheumy said that you are hyper (do you have your labs you could post?) you really need to be seeing a good endocrinologist.

Most of us who have thyroid problems exhibit with both hyper and hypo symptoms.  Yeah, it's maddening.
