What does this mean? And should I worry big time?
There is no thyroid nodule identified however there is a right parathyroid nodule identified measuring 1.4 x 6 cm.
Nuclear medicine parathyroid study and/or MRI of the neck is highly recommended

There isn't really enough information to comment fully. Most of us have 4 parathyroid glands that reside directly behind the thyroid gland. The only relationship the parathyroids have with the thyroid is their location.
