What is with my thyroid ? Low ft3, low ft4, high tsh

I couldn't lost my weight for a long period of time so I have done some blood labs. Can someone tell my something about my results ? I am man, 23 yo , 186 cm, 86 kg.

TSH - 3,33 (norm 0,27 - 4,2)
FT3   - 2,11  (norm 2,70 — 4,30)
FT4   - 1,04  (norm 1,00 — 1,70)
ATG   15,35 (norm 0 — 115)

Also i feel very often tired, and i cant sleep at night. Sometimes i have depresion, cold hands.

Your doctor is wrong, and apparently knows very little about hypothyroidism.  You can try to help the doctor better understand diagnosis and treatment of hypothyroidism, by giving him a copy of the following link and asking to be tested for TPO ab  to see if Hashi's is the cause of your low levels of FT4 and FT3.   In addition since your FT3 is so much lower  than FT4, there may be a conversion problem.  So, if possible, you should test for Reverse T3, and a FT3 from same blood draw.  The ratio of FT3 to RT3 is an important indicator of tissue thyroid levels and effects.  You can also read about that in the link.

Since hypothyroid patients are so frequently deficient in Vitamin D, B12 and ferritin, you need to test for those and then supplement as needed to optimize.  D should be at least 50 ng/mL, B12 in the upper end of the range and ferritin should be at least 100.  All 3 are important for you and ferritin is double important for good conversion of T4 to T3.  

If your doctor is unwilling to do these tests and reconsider treatment with thyroid med, then you will need to find a good thyroid doctor that will treat clinically by testing and adjusting Free T4 and Free T3 levels as needed to relieve symptoms, without being influenced by resultant TSH levels.  Symptom relief should be all important, not just test results.

Thx for that. I ve done ATPO and it was 36,1 (norm <60) so it looks like it's not hashimoto. So i will try to find new doctor, and i will suplplement with Vitamin D, B12, Ferritin
Some good supplements for ferritin are ferrous fumarate, ferrous bisglycinate, and ferrous sulfate.  Along with that you should take some Vitamin C to help prevent any stomach distress from the iron.

The symptoms you have are frequently related to being hypothyroid.  Also, your Free T4 is at the very low end of the range, which is too low for many people.  Along with that your Free T3 is way below range.   Free T3 is the biologically active thyroid hormone that is metabolized by all the cells of the body to produce needed energy.  Your TSH is within range, but that does not preclude hypothyroidism.  I am not sure what the ATG test is.  If it is the test for Thyroglobulin antibodies, then it was negative for Hashimoto's Thyroiditis.  The other test that shows up Hashi's more frequently is the Thyroid Peroxidase antibodies test (TPO ab).  

So your symptoms and test results are strong evidence of hypothyroidism, and the need to be on thyroid medication.   What has your doctor said about your symptoms and test results?

Thank you for advice. My doctor said that everything is good. And there is no need to treatment. I also have to add that in the past i lost almost 30 kg i very low calorie diet, so maybe Free T3 conversion is broken because of that ?
