thyroid ultrasound suggestive of carcinoma

I had a thryoid ultrasound on 4/27/10 it shows right lobe ha homogegeneity to the tissue, slight enlargement both longitudinally and transversely with a small complex nodule present laterally. The left lobe of the thyroid shows a comlex nodule with ahlo that measures 26.2mm by13.5mm.
ASSESSMENT:Complex nodule left lobe of thyroid suggestive of carcinoma Recommend FNA

I am scheduled for the FNA on June 14th it seems a long time to wait has anyone else had this type of report.I am worried if it is cancer it might be spreading i have had severe cervical neck pain and i feel like ai have a lump in my throat when i swallow. Also some ear pain on the right side. Can any one make any sense of this report?

If nodule has no internal blood flow or micro-calcification, the outcome is most likely benign. In this case it is monitored.

Thank you. I had the U/S yesterday and equally did a biopsy of the left thyroid gland, awaiting the result. My TSH =  1.55 T4 = 1.04. What next, l am anxious.

i just had an MRI without contract because of a left arm.shoulder pain only to found out that the A 2.0 AP x 2.3cm TV x 3.9 cm SI cystic lesion located in the right thyroid  gland. The impression further indicate differentials that include cystic thyroid nodule, veno lymphatic malformation, brachial cleft cyst. Studies suggested dedicated imaging of the neck with contract.

Left thyroid gland also show a nodule that may include abnormal lymph node.

I am referred to an endocrinologist, at this stage advice on what my next step should be.

"normal" TSH ranges from 1-5 - ish, but most of us feel best at 3-3.5.

Hypothyroidism can do that...muscle spasms are common when your TSH is high.

You can live just fine without a thyroid, most of us do, if you don't take it out, you are probably just delaying the inevitable.  The nodules won't go away, and they will grow.  The irregular borders could be of concern, but again, my nodules were nice and round, they were cancerous.

i do not wish 2 surguries on any one including myself wow that would be frustratin.I obtained a picture of my ultrasound it shows alot of of the white microcalcifiations and the orphan annie eyes it is large to me 2.6cmthe other side being elarged with inhomigenity an a complex cyst i am tempted to tell them regardless of fna results .i opt for t thyroidectomy it all sounds confusing.I would hate to leave it in to have it grow larger and spread if they can;t diagnose with fna.I have spasms in my neck on the right side i have been to the pain dr and had facet injections with no relieve i take muscle relaxers with no relieve i somehow think it is connected

Probably the halo caused concern, there are certain things in an ultrasound that MAY indicate cancer, but there is NO difinitive way to tell unless they remove the nodule and biopsy it.  Blood flow is a concern, cancer needs a blood suply to grow.  Calcifications are a concern, shape matters (nice round nodules are less likely to be cancerous, but in my case that was not true...mine were nice and round and I still had microcarcinomas in them), irregular borders and jagged edges are more likely to be cancerous.  There are so many factors, and none are a sure bet...Get the FNA, you might get lucky and get a diagnosis, then again it may just frustrate you more...I had my ultrasound in Dec '08, my FNA was in Jan '09, I did not decide to have a lobectomy until April, then in May they found cancer, so I had a second surgery.  I was just going to have the side with the most nodules taken out and hoped to preserve 1/2 of my thyroid, but that didn't happen.

The waiting is the hardest part and having cervical pain and muscle spasms for 3 months makes even more suspicious so if FNA comes back inconclusive you think i should have it removed being it is 2.6cm?Just to be sure it isn't cancer since it is hard to diagnose i wonder why the endo dr told me suggestiive of carcinoma.I almost wish he wouldn't have!It would be easier and glad to find this site! Thanks

Yes, the waiting game is the toughest...if there is a carcinoma, thyroid cancer grows very slowly (unless it is medulary which is uncommon) so waiting will not cause any more problems.  I also had trouble swallowing, hoarseness, and hypo symptoms.  I had four nodules, biggest was 2.1 cm.  My FNA came back inconclusive, which is very common.  The actual report said "benign follicular clusters", but I did have cancer.  Thyroid cancer is VERY hard to diagnose...only if you get the right cells in the FNA (like my sister did, first FNA gave her cancer diagnosis) can they make a diagnosis.  Get the FNA, but keep in mind that it may not give you the answers you want.  The only sure way to get the correct diagnosis is to remove it and send it to Pathology.  Sorry!

It is NOT possible diagnose cancer by ultrasound only!!
the nodules with calcification, irregular borders and internal blood flow are having 23 % odds of being cancerous. The ultasound guided FNA is 80 % accurate to differentiate between cancerous and non cancerous nodule.
