Help! What should I do for him??????

Someone I'm close to is a heroin addict. So for the last two years he's been having bad hard hits to the head. Kicks, falls several times! Memory lost,hallucinations, paranoid, no balance is so off. When he's high its worst. Cant remember what happened seconds, minutes ago. I mean worser. In January. He fell in the basement which its concrete!!! His whole body weight went straight to the floor. His face hit that Floor, the middle of his forehead had the biggest knot ever, I was so scared and others too. He's constantly fallen more since. Help!!
