Breathing Difficulties

Hi, This is going to be "HOUSE" like question.
One of my family relatives started having chest pains, difficulty breathing and choke like feeling about 2 weeks ago. It seems to be happening more often when he tries to drink or eat. But not always. He is fat, with mainly stomach fat - 160cm / 120kg aprox.
His medical history includes heart issues and a long treatment - bypass surgery, and since then 10 catheterization procedures.
in the last week he has been hospitalized, were he was going through examinations such as echo-heart, catheterization, chest CT, blood tests etc.
Other than high blood-pressure, reaching 240/165 max and after few days of medication stabilizing  at 120/70, all other parameters seems normal and even good - no clugged arteries, no PE, no troponin.
He is being given Omeprazole based treatment, with other cholestrol, blood pressure and more meds.

Waiting for brain storming...

Chest pains, difficulty breathing and choke like feeling can be due to exacerbation of gastric reflux or due to hiatal hernia. An endoscopy may help clear this diagnosis.
Apart from omeprazole, lifestyle changes will need to be incorporated. These include: Avoid heavy meals and eat frequent small meals. Avoid too much of caffeine, tea, smoking, fried food and drinks both alcoholic and non alcoholic fuzzy ones. Avoid heavy exercises within 4 hours of a heavy meal. Raise the head end of the bed by pillows to 30 degrees. Avoid lying down for least 2 hours after food.
Pulmonary hypertension is another possibility.Asthma like condition should be ruled out by pulmonary function tests.
