Hi
Thanks for writing to the forum!
Rouleaux formation is seen in infection, inflammatory and connective tissue disorders and in certain cancers. This increases the ESR and since your neutrophils are high chances are that there is an underlying infection. PID (pelvic inflammatory disease) is often not thought about and should be looked into. A UTI should also be looked into even if there are no symptoms. A kidney stone can also be a possibility.
Hope this helps. Do discuss these possibilities with your doctor and get yourself examined accordingly. Please let me know if there is any thing else and do keep me posted. Take care!
Thank you for your reply - it is really nice to get an answer back.
I am really perplexed that if I have an infection, how come I don't know where, or feel sick etc?
SOme of my other blood were PCV 0.36 (0.36-0.47 normal), MCV 82.2 fl (80-100 normal), MCH 27.1 pg (normal (26-34), MCHC 330 g/L (300-360 normal) RDW 13.9% (11.5-15.0 normal( Plt 308 x 10^9/L (150-450 normal)
My thyroid has always been normal as far as I can remember. This time they only did T.S.H = 1.1 (0.4-5.5 normal)
NA was a bit high normal at 142mmol?l (135-145 normal).
K = 4.7 (3.8 - 5.2)
Cl = 103 (99 - 110)
Bic = 21 (22 - 30)
Ure = 4.3 (3.0 - 8.0)
Creat = 72 (50 - 120)
Ca = 2.24 (2.15 - 2.55)
Phos = 1.24 (0.70 - 1.30)
TProt = 77 (63 - 80)
Alb = 44 (36 - 46)
Glob = 33 (25 - 36)
Bil = 6 (1 - 24)
ALT = 23 ( 1 - 45)
AST = 27 (1 - 45)
GGT = 25 (1 - 55)
ALP = 79 (30-120)
All my iron-type levels are lower than they have been and I don't menstruate (hysterectomy) so the Dr does not understand why it is low. My brain scan showed a small amount of calcification in the pineal gland area which he said was normal or at least, not unusual (and I know this is not the same as the adrenals that are like on top of the kidneys.)
If I am over weight but have a low eGFR - does that mean I might be becoming diabetic?? (My fasting glucose was 4.4 ( 3.0 - 5.4). I have occassionally had hypoglycemia.........probably just diet related ( i.e skipping breakfast).
I have had postural hypotension for a long time, but pretty mild. But it has just gotten worse & worse in the past 2-3 months. I has a panic attack last year, but being fat & over 40 everyone got all flustered (I had chest pains). They did about a million of those blood tests - Troponin....yay! I remembered....they were ALL normal. ECG was normal except the stress test one which showed - surprise surprise...I was unfit. I get ectopic heartbeats from stress sometimes, or caffeine, but have had that since I was a kid & always ruled benign. They even did an angiogram, and apart from the faact my heart was thumping a million miles an hour cos I was sooooo scared - it was fine & clear. I was so needing to be convinced the Dr finally took me into the room with the monitors & things and showed me my heart & the vessels & the blood flowing nice & smoothly & then he showed me some other patient's (done same day ) which was all jambed up and how they opened a stent thing in it. So....as of about 10 months ago, at least my heart & major vessels seemed healthy!!
Is there any diseases or anything weird that can cause my abnormal blood results - the ones that are abnormal. I had something else done about 6 months ago....that's right - I think it was a CPR test? That was like 19 (and say, anything under 20 was normal.) Something like that anyway.
I just hate it when the Dr's don't explain things. they may know what they are looking at, but that doesn't help me out any!
Thanks again for your reply Jules.

An afterthought: if your doctors think this may be a bacterial infection, they may want to put you on a heavy course of antibiotics to see if this helps with anything. I'm not sold on it being bacterial, though...but the neutrophilia is suspicious.
It appears that most of your bloodwork points toward an infection of some sort. Neutrophils are small white blood cells that naturally circulate in your bloodstream gobbling up debris, and are especially useful in fighting bacterial infections. The white blood cell count is classically elevated in patients fighting infection, so you are probably fighting some kind of infection (the elevated neutrophils all but confirm this). The ESR being so elevated is indicative of inflammation, though I have heard from my doctors that an ESR up to double your age is considered normal. For me, this would mean that an ESR of 44 is "ok" but not optimal by any stretch of the imagination. Currently, my ESR is 2, just to give you a reference point, but if you're much older than me (I'm 22), your ESR is expected to be a bit higher. The GFR refers to your glomerular filtration rate, which essentially measures kidney function. Typically, the GFR is decreased in patients suffering from chronic kidney disease, and increased in obese patients and also as an early finding in patients with diabetes.
I know that orthostatic hypotension is often an issue with patients suffering from some sort of adrenal dysfunction. Have you had any hormone panels done, particularly any that include thyroid and pituitary panels? I also wonder if the lowered blood pressure is a sign of adrenal disease, or of blood vessel inflammation.....
If you were borderline anemic, your red blood cell count, hematocrit, and hemoglobin would be low-normal, if not already below normal. Have you been able to compare this bloodwork to previous bloodwork? Just because you have a low-normal value (i.e. a 7 on a scale of 6 - 12) doesn't mean you need to worry about those values dropping or being low.
My Nutrophils were 8.4 or 69%, NOT 60% as written in my entry.
I also forgot that the hypotension is why I went to the Dr in the first place - I have been dizzy and sometimes get postural hypotension where I almost faint, or am so fuzzy headed I walk like a drunk and I have momentary amnesis - ie. I forget what I was about to do, or why I was walking that way etc. It clears rapidly once the blood reaches my head and I have had a head CT scan which was normal.
Thank you
