Debilitating tension/pressure feeling in head when standing up.

I've had this on and off for over 5 years now, I don't know what to do.  Basically the feeling I get is like an extreme tension, throbbing with my heartbeat (but no pain) pulsating pressure in head when I stand up from sitting or lying position.  It doesn't hurt, just feels like something inside from my neck and above is causing an unusual change in pressure that I can't tell if its high or low pressure.  It is extremely uncomfortable and I have to stop moving momentarily for my body to normalize itself, usually takes a minute or two.  Walking around is uncomfortable during that time not because I can't walk,, but because of the pressure feeling in my head.  I feel like my head is going to implode, or explode, not sure which one.  

Some days I don't have it at all and I feel normal and strong, but other days when I get this, it is a struggle to go about my daily life because getting up to do things always triggers this and slows me down/makes me feel bad.

I'm going to see a doctor again, but every doctor I have brought this up to has no idea what I'm talking about.  My blood pressure is a bit low but on the normal range.  I've even measured my own orthostatic blood pressure at home and it seems normal, no drop when I stand up in either pressure or heart rate.  

I'm at a loss and don't know what to do.  Could it just be stress?  Tension?  Something wrong with my heart?  cardiovascular system?

Once thing I noticed is that I get it really bad on days after I have worked out or done some strenuous activity.  Like cycling, lifting weights, or general tiring work.  

Anyone have any ideas what to ask my doctor to check for?  My recent blood work came back totally normal.  The only thing of note was that I had prior exposure to EBV, Bartonella, and Rocky Mountain Spotted Fever.  

The only thing of relation would be the Bartonella since I've read it can contribute to cardiovascular disease.
