Multiple unexplained illnesses

My partner will be turning 40 next month. In the 11 years we have been together she has suffered severe/chronic back pain. She refuses to exercise or even go for a walk. She says that footpaths make her back and lately leg pains worse. She is on thyroid medication for a non active thyroid.  Her periods are unusually heavy and severe abdominal pain. In addition she alternates between constipation and diorrah(?). Numerous episodes of bleeding when passing hard stools, she says tearing. To add to the mix are leg jerks especially when in bed and crazy itching-arms legs body. She has increased stopping breathing(holding breath) while asleep. It wakes her up. She has had some very bad episodes of choking on nothing, in fact she almost broke her ribs hitting a wall to try and get her breath back.
With all these things she will not go to a doctor. She says they will only look at one thing and either she is scared or in denial. Any ideas?

Sorry I forgot to add " Her medication )(THYROID needs to be adjusted , it takes time but when she gets the right dosage it will be like a new life,,,,

i hear the low thyroid , with some of the symptoms but if she goes to a dr. that in itself will relieve alot of the symptoms, she could have a heart attack if she continues like this...Get Help God Bess YOU Both
