Potentially dangerous symptoms, need help finding out what it is

First of all let me get this out of the way, I've already been diagnosed with vasovagal syncope (sight of blood, pain triggers), a slight hiatal hernia. I also smoke socially. Nothing more than 1-2 cigarettes a day a few days a week, 2yrs.

About a month ago I had a vasovagal attack (and I've had many), that was different than most. The symptoms that brought it on were as follows in descending order of occurrence: sharp and sudden pressure like pop in area under bottom left rib, almost right after a headache in my left temple, slight pressure in my chest. I went to lay down and felt my vagus nerve responding to the pain and blacked out. Apparently I was out for 3-5 min. When I came to it took longer than usual for me to get my motor control back and realize where I was. Usually when I have one of these attacks its only 20 sec and I'm fine. This time it took around 5 min. After that I couldn't stand or sit up long without feeling faint. I also had the most insane thirst for water ever in my life, and drinking water didn't really cure that. I had a smaller version of these symptoms tonight while driving, I willed myself not to respond to it however. This time there was also slight lower back pain. An hour later and I still have a slight headache on the right side.

One more note I've had neck and foot pain for a month prior, neck pain due to an unergonomic workstation. After my blackout my neck pain was much more pronounced, and I could almost feel the blood going through it. I went to the ER that night. EKG was fine, blood sugar normal, neck xray normal, slight dehydration (maybe because they refused to give me water). Any answers or thoughts would be great,

I'm pretty sure that my hiatal hernia was the cause of it, it can stimulate your vegus nerve of which mine is over stimulated already.

um i've herd alot of these kinds of things with sevre nerve damage! I have Intractable pain diease and though thats alot different i had several tests done for the last 4years Before Doctors found my problem! i was givin Lyrica & gabapentin two medicines that i reserched Be for even takin just to make shure they were safe for me & well alot of personal experiances i read about had people with similar experiances without those meds and the meds seemed to help them! But i'd say the ciggs have nothin to do with it!  you need to get checked for nerve damage and or nerve syndroms by a specialist  also maybe a pain management doctor!  also not to scare you but you should get checked for pre-epilepsy! eather way you need some tests run for whatever this crazyness is so you can figure it out and maybe find some meds to help you conquer it! sorry I wasn't more help! But I hope it's nothing too crazy! take care peace

I forgot to mention that these episodes generally occur when I am in a seated position.
