Unexplained symptoms developed after accident.

Hello,

I am 24 years old European. On 20th of August I was in a motorcycle accident, where with approximately 120 km/h I fell and slid on the asphalt. The hit was primarily taken by my right shoulder, back and then left shoulder as I rolled.

Initially I was able to move and do everything from the adrenaline, but 5-10 minutes the traumas became apparent - specifically extreme pain between right shoulder blade and spine. I had small dent on my helmet, which means that there was no significant hit to the head, there was also no neck or head pain in the first 24 hours. On the third day they've sent me to recover at home as no damage to lung or internal organs or blood vessels was noticed.

I was send in to ER where they did CT of Chest and Abdomen and X-rays of my shoulders. They found broken right transverse process of T1, which I was told was no problem, as the injury was to the end of it and there was no dislocation. I also broke my first left rib completely slightly behind the collar bone. I had limited range of motion in right and left arm for 6-7 days and the pain between my shoulder blade and spine was my main problem for 7-10 days after the crash, as it caused me difficulties standing up and moving. It was limiting me from moving as I was getting nerve pain/muscle spasm or whatever. I never lost consciousness and was generally feeling alright beside the physical pain and the hard time moving.

I slowly got better in the next week, I was able to get out of bed, I felt totally normal and was waiting to get better, while managing to do my regular tasks on my computer, follow up on university courses online and etc. However, as I got better physically and regained my range of motion I started feeling weird symptoms around the 10th day which I believe are getting worse.
Some of them are:
Gradual decrease in the amount of pain I am feeling overall, I feel pain, but I can withhold extreme nail pinching and etc. while at the same time I have sensation of hot and cold, touching, can distinguish sharp from dull objects everywhere on my body. Pulling hairs out also doesn’t cause me as much pain as before. Also I rarely get urges to urinate in the morning anymore - I can wait for long time.

I feel my muscles strange, like they are not mine. I cant recognise when exactly I get fatigued. For example I am talking on the phone for 1 hour, and only after I move my hand I can feel slight burning sensation in my shoulder, where before I would have felt it gradually. I have the strength to squat on one leg with both legs, however even though I mentally feel that I have strength in my upper body my ability to do push ups is limited to maximum 1-2, where before the accident I could do 20.

I don’t feel hungry or thirsty. I am drinking water because I know that I must, I can go without eating for 24 hours and eat out of the idea that its not normal to not eat instead of from hunger. My reward system is totally out of balance, my weight usually roams between 98-102 kgs and I have had a huge appetite and sense of thirst before, so having those sensations dampened is weird, strange and extremely scary for me. My stomach seems a bit out of order but its probably due to the low food intake.

I developed extreme fatigue, where I can sleep for 9 hours and wake up tired, not having motivation to get out of bed, even for playing computer games, which I could do for 16 hours multiple days in a row and gave me really high pleasure. I am having trouble concentrating, because I am all the time worried what can be wrong with me and therefore cant lead whats defined as “normal life” for me. Also don't have motivation and can't keep focus normally in my university, where compared with the first 10 days after the injury I indeed had high motivation.

I met several neurologists, who could not find any significant or important pathology and said that further testing is not needed.

Despite that I decided on my own to do tests just to check for possible problems.
I did
Head Mri - no problems
Cervical spine - no problems, except some initial stage of disc herniation between c4-c5, for which I was told had no clinical relevance, and lowered water content of intervertebral space of c1-c2-c3
Thoracic spine - no problems
Full Blood test - 21 parameters all normal
Asat, alat etc all in norm
Blood sugar normal.
i even did STD tests and they’ve shown nothing
No viruses or anything in urine
The only thing which was slightly higher was the total billirubin 29 with parameters 3-21
Several blood pressure tests and EKGs - 120/80 60-80 pulse

I haven’t tested hormone levels so far, but I am thinking about it.

Doctors don't take me seriously as no examination has shown anything out of the ordinary and they say that there is no problem.

I am desperate, some doctors say its pstd, anxiety and what not, but I really don’t believe they can cause SO exaggerated physical symptoms.

25th September I got like super dizzy with lump in neck, couldn't force myself to eat or drink. At the time I prefer laying/sitting as it makes me feel slightly better.

This became really long so I will add, if I remember something else, later.
Please if somebody experienced something similar or has some explanation to this let me know.

Possible culprits are Post  Traumatic Stress Disorder and perhaps low production of the antidiuretic hormone  vasopressin due to dehydration,
If I were you I would have a complete hormonal panel, including a functional 4 X cortisol stress test (saliva)
Best wishes,
Niko
