What’s the cause?

Hello, for the past couple of months, off and on(but mostly on) I’ve had this constant dizzy-headed or loopy kind of feeling.
Along with brain fog and a weird sensation when my eyes move, almost like they’re tired. Sometimes the “dizzy” part is really strong, enough to make me wanna sit down and let it calm.

Early this year I had a heart scare, as I developed a lingering chest pain(still have it today just not as bad) and thought for sure my heart had something to do with it. Let me say I’m 27, almost 28 in fact, and more commonly heart problems tend to happen at a later age. So you can imagine the fear I was going through, I had my local hospital run several tests which include; two X-rays, a scope down my throat to check my stomach, an echocardiogram, a total of 3 EKGs, two cardiac blood work-ups (both of these from ER visits) a 48-hour holter monitor(as I felt my heart skip time to time) a ct scan of my chest.
All of which, has returned normal. The monitor did pick up PVC’s but I was assured this was normal as everyone has them occasionally some just don’t feel it. (although I’ve never felt them til November of last year) I feel them everyday now, roughly 20-30 times a day. I’ve kinda learned to live with them, but they used to scare me.

Other than that in January I was put on Paxil for this anxiety I felt, and since I thought i was suffering side effects a month later I asked to get off it. Those side effects I thought were;
Dizziness
Brain fog
Sometimes blurred vision, (like my eyes couldn’t focus fast enough on close objects after looking afar or vice versa)
Nausea

Once all my tests had came back in the clear; I tried to live my life again, and for a while (except for the occasional loopy headed feeling) I was doing fine. Then golf season started back up in April and I went golfing one night with a buddy and all I can remember really is my back and my chest hurt from swinging. On the drive back, I got crazy dizzy and had to pull over to the side of the road. Those dizzy spells have been going on for a while now, and I have no clue what’s happening. Everyday it seems like I have a headache, it’s not always in one spot, it tends to move around but it feels mostly behind my eyes, sometimes left sometimes right. Every time I chew food, I feel like my eyes need to close, they don’t do it on their own, I make them close cause I get the feeling I need to - like when a bright light hits your eyes and you squint kinda feeling.
This all happens during the day, cause every morning I wake up I feel practically normal, until about an hour or so later when the loopy feeling starts and I’ll usually get a headache around noon, take some OTC meds, and it’ll be gone by 1 ish, til about 6-7 at night.

Insight. I hope whatever’s going on I can figure it out cause I’m losing my mind. It’s changed my social life for the worst... and I use to be such a social butterfly.
