cause for recurrent fever?

I'm 19 years old and have been getting a 104 fever every 28 days like clockwork for 4 months now. Blood tests, throat cultures, etc. always reveal that "nothing is wrong with me". The fevers are accompanied by an incredibly sore throat, pain with swallowing, swollen lymph nodes, body aches, chills, fatigue, and a general pain (last time it felt like every hair in my body was being ripped out when i tried to change my clothes). the fever and sore throat lasts 3 days, i feel better/super healthy for 25 days, then again comes the fever.
Any ideas?
The doctors here on campus (the only thing i can afford right now) are not the most intelligent people, i'm frustrated.

A cyclic pattern of fever is seen in EBV or infectious mononeucleosis, malaria, autoimmune disorders like rheumatoid arthritis, SLE, otitis media, endocarditis, IBS, STDs and cancers. Rare causes of unexplained fevers include (but are not limited to) taking certain medications, adrenal disease, hematomas, hyperthyroidism, pulmonary emboli, and previously undetected thrombophlebitis You can go over all these possibilities to see what has not been tested for.

Are these conditions somehow related to your menses??? I mean do the symptoms appear right before it?

Has this happened for a long time or just started recently?
