confused about echo stress test

I am 5' 7". Last year in 2008 I weighed around 155. I started gaining weight when I broke my foot and started taking Methadone. I started retaining fluid also. I gained 35 lbs. in 6 mos. I had pitted edema in my legs and feet and I could really tell in my upper stomach. Test were run, no prognosis, just gave me lasix. That weight stayed with me and the fluid has come and gone, better with my feet propped. Now this year, in the past month and a half, I have gained an excess amount of fluid. I went from 190 to 22? with about 2-3 weeks. It's in my legs and ankles and feet. I am really tired, have insomnia with daytime sleepiness. I went to the Dr. the 1st time about 2-3 weeks ago. She did blood work, everything came back w/in normal range. I went back a week later and she gave me hydrochlorothyizide. It didn't help. I went back at the end of that same week and seen someone else in the same practice. She did a chest x-ray and found fluid in my lungs, gave me a shot of lasiks and a scrip for furosimide, that was on Fri., Sun. morn I woke up so swollen my eyes were almost swollen shut. I couldn't bend my ankles, they no longer existed. I dealt with it that day and went to ER early Mon. morning. They done an echocardiogram and blood work and said- within normal limits. So I went back to the Dr. on Tues. morn. I seen a different dr. in the same office. He seemed really concerned. He ordered a sleep study which was done on last Thurs. to make sure i don't have sleep apnea, nothing wrong there. I had an echo stress test done yesterday. Before I tell this let me just note that my normal BP has always been around 145/78. Since all this edema started a couple of weeks ago it's been around 128/89-96. Before the stress test it was 128/89. After the stress test it was 145/74. Why did my top # go up and the bottom down? He said he was going to send me to a cardiologist, but he had to get a second opion. He paged the cardiologist on call and I guess he said no. He put me on a new fluid pill that don't mess with your potassium because of heart reasons. I'm confused. He done more blood work,-- a sed rate, tsh, urine, and metabolic. I have no faith he will find anything considering. He also said he didn't think my edema is vascular, it's interstitial maybe? Not sure if that's the word he used. I also am having shortness of breath really easy. I really need a medical point of view from someone else. I know this is alot to take in, but could you maybe help me? Give me an idea of maybe what it is? Suggest what the dr. should do next? I am at my wits end. PLEASE HELP ME!!! THANK YOU FOR YOUR PRECIOUS TIME.

i went back to my dr. he sent me to a cardiologist. i have to have a left and right heart cath done on dec 1.

Thank you so much for your information. I just got more out of you than I have all 4 of the Dr.'s I have seen. I have been doing research on my own and it seems like I have alot of symptoms that someone that has ischemic cardiomyopathy. The top of my abdomen is really swelled and firm with fluid. My legs, feet, and ankles are too. My legs also pit really bad. What is your opinion?

Generalized body edema (fluid retention) can be very serious. It can be due to severe anemia, a failing heart, serious kidney disease, very severe anemia, liver failure, polycystic ovary, low protein in the blood, electrolyte imbalance and disorders of adrenal gland.
I understand you have gone through extensive tests. See if kidney and liver function tests were done, hemoglobin and iron checked, serum albumin and globulin done, and serum electrolytes also.
As for the results of the stress test, something is hampering the normal response to exercise. A cardiologist must be consulted regarding this. It can be hormone related too hence polycystic ovary and disorders of adrenal gland should be looked into.
