Chronic Pain in Penis and Testicles

I just turned 40. I have had chronic pain in my penis and testicles for 8 months. It started immediately and intensely one day after I had just gotten a new Cialis prescription. I took one and masturbated, which I hadn't actually done in about a month or two (was completely celibate). I had an intense aching pain in my penis afterwards. The pain has continued for constantly for 8 months. I also feel like an uncomfortable "pulling" feeling when I get an erection. Note that I haven't had actual sex with anyone in at least 4 years. I saw a urologist initially. He checked for an infection and didn't find one. He did a scope up my penis but didn't see anything but diagnosed me with BPH. He prescribed me antibiotics, which didn't solve anything. He recommended I see a neurologist. I was already seeing a neurologist for pain in my arms and legs. He did EMG for that and found no problems. He prescribed me Gabapentin which seems to help arms and legs but not pelvic pain. He referred me to another hospital 4 months ago which I haven't been able to make an appointment with due to COVID-19. I finally have an appointment with them the next week to see a neurologist. The best diagnosis I can find is pudendal neuralgia. Is there anything I should request for them to examine? What should have I have examined? What tests should I have run? What is the likely cause and treatment? I can't live like this. What is the significance of this starting immediately for the first time after masturbating on a new Cialis prescription?

Wow - this sounds like a very difficult issue, both from the standpoint of the length & severity of symptoms, but also from a medical perspective as far as being able to properly diagnose & treat this. I rather doubt anyone here - other than an actual medical professional w/ experience in urogenital health - would be able to help. Although I guess if someone reading your post experienced something similar, their advice could be helpful.

I do have a couple of questions:

1) Why did you get a Rx for Cialis? Were you experiencing ED issues, or was it just something where you felt an added 'boost' to your functioning would help?

2) When you took the Cialis, did anything feel different? I mean I'm sure it facilitated a better erection, but did it feel especially hard, maybe to the point it almost felt painful? Some guys who take some of these ED meds do risk getting Priapism - an extremely hard & persistent, often painful, erection that doesn't subside after sexual activity. Did you have any issues with an erection that took a lot longer to 'deflate? I've heard of younger guys who don't really have ED using these drugs so they can have 'prolonged' sex with a partner & get another erection very quickly after the initial session. So what I'm asking is, when you masturbated, did things feel pretty normal? Did you maybe engage in a 2nd or 3rd round following the 1st? If so, that could be part of the issue. I'm just speculating here, really don't know, but I thought it's worth asking...

In the end, I think it's going to take good medical care, some tests & a good Urologist to figure out what's going on. Good luck with this & check back here & let us know what you find out. I'm sure others can learn from your experience & also many of us are prbly just curious to know what's going on with this unusual case...
