Enlarged Bladder & Prostate

I am a 65 year ond male i am schedualed for a TURP surgery on 9-2-08 my question is 3 months ago i was retaining 1400cc of  urine and could not pass urine, after cathing for 3 months my bladder has shrunk to around 8-900cc, now if i hold off the surgery until my bladder shrinks to a normal size, would it be better than going ahead with the TURP,  or would i be worse off if i had the TURP surgery with a bladder that i could not empty, What is a normal size bladder and how mutch will it hold.I have a appointment for pre-op Monday aug -18-08 could you please let me know what you think God Bless .

Hi,

PVP has been proved effective in treating large prostates. Sizes up to about 200 gm can be handled in a single surgery. For sizes larger than 200 gm, multi-staged PVP will need to be planned.

Additionally, the symptoms of pain and discomfort would take a week or two longer to subside.

Complications are fewer and hospital stay is significantly shorter than for the TURP.

If you plan to have PVP done, it would be good to check on your doctor's expertise in this regard. Especially regarding experience with large prostates.

I have a Prostate the size of a Tangerine,
Would the PVP sergery work well with this size of a prostate.

Thanks for the info,
Would the PVP sergery worl as well as TURP even tho the Prostate is very large, Or does the size of the prostate determin which sergery you use. Thank you Martin

Hi,

The normal capacity of the human bladder is 400-600 ml. Individuals can develop the urge to urinate at lower levels of filling and the bladder can expand to hold larger volumes of urine without pain in some individuals.

If the bladder obstruction was caused by an outlet obstruction due to prostate hypertrophy, it is necessary that some intervention be taken to relieve the pressure and tissue blocking the outflow of urine. These measures can involve medical and surgical management.

In your case, as the obstruction is complete and persistent, it may be a good idea to have aggressive intervention to correct the problem. You can discuss options of PVP (photoselective vaporization) as an alternative to TURP as the recovery is faster and so is the time duration until catheter removal.
