How can the doctor be sure it’s Bacterial Prostatitis without testing

Rather long story, I had sexual relations with a girl 6/19-6/21 rather rough but on 6/23 I started getting pain in my testicles and tip of penis, no burning when I pee just pain. I looked inside my meatus or head of penis and it was red and purplish looking. The pain was so unbearable I went to the ER, they tested my urine and found some blood and some bacteria. I told them my back story and they said it’s an STD, but not before they did an ultra sound of my testicles. I was told I have Epipydmtits. They gave me a shot in my butt and one pill to take while we waited for the results to come back. A few days later the test came back negative and I felt somewhat better, i went to my regular doctor and was given doxycycline for 10 days and I felt better, but I lost 10 pounds in 10 days. I then went to the gym and worked out after finishing the doxcy, that night the pain in my penis and testicle came roaring back and I was back in the ER, I was giving a urine and ultrasound test again and told to visit a urologist. I did the next day, the  urologist looked at my meatus (opening of my penis) and said yep it’s gential herpes. He was sure of it because he’s seen it a million times before. He did a literal10 second exam on my prostate and said it felt fine although I told him I had some pain after he pushed on it. I was given Valtrex, and told to start taking it. I was so sure it wasn’t gential herpes so I didn’t take the pills. I found out a week later it wasn’t gential herpes. So i was back to square one, I went to my family doctor and I took an HIV test because I had lost so much weight, which came back negative although I will retest again in a few weeks. My doctor sent me to another urologist. They tested my urine and it had some ketones in it and trace amounts of bacteria in it. After talking with her for awhile she prescribed 250mg of Cipro for 28 days. I am currently on the Cipro now and I feel much better but I still have some pain after sitting for to long or randomly one testicle will hurt but it goes away when I adjust my testicles. As far as the meatus pain that’s completely gone. My question is how does the urologist know this is a baterical prostatitis and not something else with no real test to prove it. I’ve read some pretty nasty stuff about Cipro and on day 6 I’ve had diarrhea all day today. I guess I’m just wondering is this Prostatitis or something else, this came completely out of the blue but after my sexual encounter. For the record my partner showed me her test results and she was clean as well, so I’m thinking this wasn’t caused by an std.

Did you ever get relief from this issues you were dealing with?
I am going through pretty much the same thing been going on for 8 weeks nothing has worked!
