Insensitive Penis / Clear Fluid

Hi there I have a couple of related problems.  Any assistance would be greatly appreciated.

Details: Uncircumcised 23 year-old male. I am 6ft and 85kg.  I don't smoke.  I drink once a week- usually a couple of bottles only but would say that once every couple of months I would binge drink.

1. I believe I have an insensitive penis.  I started masturbating around the age of 16 years-old and as I've got older i have only really climaxed after vigorous masturbation.  My ejaculation otherwise is normal.  I am now in a very loving-relationship and have been unable to climax during intercourse (over  4-month period).   I have tried abstaining from masturbation but this has not assisted.

2. When I do ejaculate with my girlfriend (only by oral sex), there is no sperm - only the prostate fluid is ejaculated. There is usually quite a lot of this.  

When I climax with my girlfriend I do have an orgasm and it is lots of fun, but what to know what is wrong with me? Have I done any permanent damage?

I am concerned about the long-term future i.e. conception.

a couple of people messaged me saying they have similar problems but no one able to provide answer.  At least 3 people would be grateful if one of the Docs had a look at this post.
