If I were in your situation, I would seek out a physical therapist who specializes in pelvic floor physical therapy. Usually they work more with women, but some of them also work with men. If I couldn't find someone locally I would, if I could, travel to or consult with some professionals in this area, such as the clinic at pelvicpainrehab.com. (I have not ever been there and have no affiliation but they seem like the super experts on this stuff).
Kegels can help or be harmful depending on what is going on. Some things can be too clenched already and in that case, kegels can make things worse. If I were you I would probably also have anxiety about that part of my body, and that wouldn't help either!

Wow - sounds like a difficult case & definitely very troublesome. I was gong to ask about the possibility of a hernia, but you said that’s been ruled out. However, somehow I think there’s still a chance that’s the issue. I’m not a Dr & even if I were, I think most Dr’s would not want to diagnose you without a personal visit & thorough exam. Have you seen a Urologist about this? I think this is going to take someone highly specialized in this area to get to the root of the prblm. Don’t give up - if one Dr can’t help, keep trying & ask for referrals to specialists. As for the Kegels, they prbly aren’t doing any damage, but if they seem to be making matters worse, I would at least back off doing them for awhile, let things heal for a cpl wks, then try again cautiously.
