Urination irregularities & need-to-pee feeling associated with ejaculation

Hello all,

I am a 23-year-old male recently dealing with irregularities associated with urination, as well as discomfort associated with masturbation and orgasm. When peeing, the stream will often (but not always) stop before my bladder is emptied fully, and I'll need to push to get the rest of the urine out. This requires a conscious effort to fully relax all the muscles in the area before bearing down with my front pelvic muscles in an attempt to empty my bladder. This will seem to empty the bladder at first, but often a lingering urge to pee will remain—like a slight burning, not painful but uncomfortable and distracting, as if there is still some urine to get out—and I’ll end up revisiting the toilet four or five times in twenty minutes, with a little more coming out each time.

My urine will often come out in a split stream or a stream that sprays a bit, and I often have pauses in flow. It will often take a long time—sometimes two minutes or more—before my pelvic floor relaxes enough to pee at a urinal if there are people right next to me or if I am anxious—and especially if I’ve had to hold my pee for a very long time. I’ve always been one to pee frequently, and have observed myself having to urinate more often than my peers.
Recently when masturbating, I will begin to feel a tension reminiscent of the need to pee arising in my pelvic/genital area—it’s hard to place exactly where it is originating. This will occur early on in masturbating, even before I’ve begun to feel pleasure. The tension, which feels similar to a urinary urge, soon leads to premature ejaculation, and the orgasm has little pleasure in it when experienced with this uncomfortable tension. Right after ejaculating, it will feel like I have to pee. I can’t say that all this is very painful, but it’s certainly uncomfortable and has taken the pleasure out of it.

Just to eliminate any suggestions of STDs, I am a virgin so I doubt very much that I could have an STD. I’ve read that frequent masturbation can lead to problems like this (something which doesn’t sound very scientific), but I am not a frequent masturbator—I masturbate 1-2x a week. I’ve finally reached the point at which I’ve conquered my social anxiety enough to date and have a relationship, and now I’m faced with something else to ruin my potential sex life :/

I at first believed that my symptoms were due to tight/tense pelvic muscles, and a few years ago I had similar symptoms which resolved themselves after some days of pelvic floor stretches. But this time, the stretches seem to be having no effect.

I should also mention that in the past 9 months I’ve been dealing with IBS symptoms, and have recently been dealing with flattened/thin stools. I don’t know if these symptoms could possibly be related, but figured it was worth a mention since all my symptoms are located in my pelvic region. I’ve seen a few GIs about my IBS issues, and will be going to another specialist soon.

Any ideas as to what could be causing symptoms like this? Thanks in advance :)

@impressionist, did you get any answer to the problem?
I'm now 23 y/o and facing the same problems for the last 2 days. All others physical issues seems to resemble with you to some extent.

This could be related to inflammation of the prostate - that can cause problems with urinating & also the symptoms you describe regarding ejaculation. See your regular MD, describe what you're feeling, he/she will probably check your prostate, will probably also ask for a urine specimen.

Hopefully your Dr can diagnose this properly. It could be something other than prostate, it was just my thought that could be the problem. Good luck!
