There are many causes of swollen painful testicles. It can be due to inflammation/ infection causing epididymitis, orchitis; hernia; hydrocele or varicocele or benign or malignant growths of the testis. For evaluation of the same, I would suggest you to get it examined from a urologist and get an ultrasound testis done to confirm the diagnosis.

There could be a number of causes but you don't want ignore symptoms like these.
Theres no way of knowing if its a benign condition without seeing a doctor and you don't want to take the chance of waiting and have it be something that needs to be treated right away. Its worth getting checked ASAP, it may be nothing but you have to make sure.
These are the symptoms of testicular cancer.
* A lump or mass in either testicle
* One or both testicles feeling harder or firmer than usual
* Swelling or any enlargement of a testicle
* A collection of fluid in the scrotum
* A dull ache in the groin, lower abdomen, or back
* A feeling of heaviness in the scrotum
* Pain or discomfort in a testicle or in the scrotum
* Tenderness or enlargement of the breasts
Go get checked and make sure.
