testicular torsion

I've had a sudden sharp pain last month in my right testicle and my dad took me to a doctor. He gave me some antibiotic (note that there was no injury/accident as he asked me the same question) but the next day there was swelling. So went to a different doctor and he also said could be due to bacterial/viral infection and he gave me a med for inflamnation, cortisone (deltacortil) with an anti biotic. We asked if there was a need for ultrasound but he clearly said to take the meds and have an US if the treatment doesnt work. After 2 more visits the same doctor (After 10 days) he suggested to get an ultrasound 10 days later but according to him the testicle was healing cause pain and swelling reduced. However, on 21st march when I got an US report it said I've had a chronic torsion and there's infarcted areas with "minimal vascularity in posterio-inferior " part. There's a hydrocele. And also that the echo's from both epydidimis are similar. We consulted a uroligsit and he said it was too late to save the tsticle because usually the testicle needs to be untwisted within 6-8 hours. We also consulted another surgeon he said theres no need to remove the testicle because there's only some infarcted regions and that its not fully dead. He still suggested a surgery to untwist the testicle and to remove the collection of the fluid.
What is your advise on this? Thankyou. I'll appreciate if its answered asap.

It never ceases to amaze me how much even doctors worry about their "masculinity" and let that affect their patients. Even in the face of evidence of a serious problem they prescribe antibiotics rather than deal directly with the underlying problem. I can perhaps forgive a GP who has a lack of knowledge of mens sexuality but not a urologist. They should know that torsion must be dealt with ASAP or a man is going to lose a testicle.

I speak from personal experience. In the eighties, I had varicoceles on both sides, and even my urologist was reluctant to deal with them. While I'm certain medical techniques have improved since the doctor's attitudes have not.

All I can suggest is to find a urologist who will listen to you and treat you in the manner deemed necessary by the severity of the problem. Losing a testicle is no big deal. You will be no less a man because of it.

Varicoceles are simply varicose veins. They will feel like a bag of worms in your scrotum and can be quite painful.

I will have to defer to your urologist on saving the affected testicle.

And what is varioceles? Id like to know about that just to be careful in the future.

And they also said there's no risk of infertility as far as my left one is working normally. But do you think theres any hope of getting the affected one back? after a month? Why would my reports indicate the "minimal vascularity" in some parts of the testicle otherwise?

Thanks a lot for your concern. we've met a urologist. He's suggested that a surgery be done, but before that he wants us to get some tests, semen analysis, and since in result of torsion, the affected testicle might sometimes make anti sperm antibodies which might affect the sperms produced from the other testicle, he also suggested "serum for antisperm antibodies" test to see if that is the case in my situation. But he also said that since some parts are infarcted and some are not, and if no antibodies are being made by the affected testicle, he advised to get it untwisted it but not removed yet.

And yes, he also told us that its a medical emergency and has to be treated with a surgery within 4-6 hours which unfortunately in my case wasn't done. But he's basing the surgery on the test reports, if the reports come out positive, like no antibodies and stuff, he will not remove the testicle.

And for that semen analysis, I was asked to masturbate (obviosuly in washroom, alone). But it just wouldnt erect. I thought maybe it was because of Air conditioned atmosphere, or because I was masturbating in a hospital which ive never done before, or just because I was feeling a bit shy to give my semen for analysis or idk). I asked them If I could collect the sample at home and give them tomorrow, but they said if its possible to bring the sample within 30 minutes then its okay cause otherwise the sperm germs die. Since this was not possible I just got it cancelled and decided I'd come tomorrow and try again. When I  tried masturbating at home, it erected as it usually does. I just didn't go to the point of masturbation because I have to give the sample just the next day. But I was really worried if I had lost my erection as well. Thank God it was fine when I came back home.
