Since the double dosing, I have indeed passed tissue that has contained fibroid, endometrial lining and polyp in nature. My uterus was oversized and she couldn't see past the cervical area with the saline-infused scan so none of the tissue passing has alarmed her.
I was not pregnant. My gyn office tests for that upon every visit. My PAP was clear and hormone numbers were normal. I did experience what seems to be temporary hypothyroid. I retest those numbers in a few weeks.

Well, it is difficult to say whether a poly got dislodged and the uterine surface is now bleeding. All causes of bleeding need to be investigated. A prolonged heavy bleeding of any kind from the vagina can be due to a sore or ulcer in vagina as you see in Herpes or otherwise, endometritis or infection of endometrium of uterus, cervicitis or infection of cervix, fibroids (ruled out in your case), vaginal infections, vaginal polyp, endometriosis and cancer of either cervix, uterus or ovary.
It can also be due to a hormonal contraceptive of any kind or due to hormonal imbalance due to withdrawing it. Sometimes PCOS also presents with prolonged vaginal bleed. It can also be a miscarriage. This you can confirm by taking a blood pregnancy test since the test remains positive for a month after miscarriage. If it is miscarriage you will have to go for a D and C. Thyroid problems and bleeding disorders need to be ruled out too. Also despite biopsy, endometriosis is often missed. You would need to get some other investigations done including hormone analysis, PAP smear, and maybe CT scan of pelvis. However, since I cannot examine you, nor get a detailed medical history on net, I suggest you discuss this in detail with your gynecologist. Take care!
