Need sdvice...Uterus/bleeding issues,failed biopsy and now failed D&C

I am 44, have been bleeding irregularly for 4-5 months.  I have now had a failed D&C where the Dr was unable to "look around" either.  Failed biopsies in office.  The next step would be a "hist" type test.  I just read some articles about endometrial cancers and they don't recommend procedures that could flush cancerous cells into other areas.

What could be causing this blockage to my uterus?  My IVF Dr did say he had to be careful during my implantation and use smaller instruments.  What could cause this bump and blockage?  And the bleeding.  Could my uterus not be opening up enough to allow menstrual blood to get out?  Notes below...

8/26 failed D&C, Dr was able to get biopsies but coukd not get instruments into my uterus.  She could not enter the uterus with 6mm or 5mm camera and device as there was an obstruction, and a "bump".  She said cervix dilated perfectly.

7/2016 failed in office attempt for biopsies, Dr could not get more than 2 inches deep and it was excruciatingly painful.  She said there was lots of "old blood" in there so she wanted to do a D&C.

Past 4-6 months, bleeding random times, as well as normal menstrual cycle.  Sometimes lots of tissue, much heavier and longer than ever before.  Normally I have low bleeding for 2 days, now it's 4-5 days much more blood.  Then irregular bleeds also of darn and red blood and tissue.  Bleeding post sex.

2 months ago - regular Dr conducted pap was negative, exam, and blood tests all normal.

4-9 years ago.  Diagnosed with a t-shaped uterus.  Birth mother claims no medication taken during pregnancy.  15 IVF implantations done.  None implanted.

9 years ago, ovarian drilling to clean out ovaries and old stuff, suspected PCOS, no signs of PCOS past that procedure.  Was not getting pregnant on my own.

I hope someone coukd help with ideas on next stops.  I do not want to do any tests that may transfer cancerous cells into deeper areas of my body if that is what this bleeding and blockages means.  Is this normally how endometrial cancer presents itself?  

I appreciate any assistance or info from women who have had failed D&C due to blockage.  Thanks!!!
