Fentanyl to ms contin conversion

100mg ms contin er tweice a day given to replace 3-100mcg fentanyl patches and I am still in pain why?Is dose equal

3 100 mcg patches?  No, not equal!  The 3 100 mcg patches are way stronger than 2 100 mg MsContin.  I am very sorry for all the pain you've had to endure and what has caused it all.  I'll be praying for you to have more good days than bad.

Conversions from one opiate to another are typically done reducing the new opiate dose by 20-30% for cross tolerance. Converting from fentanyl back to oral pain meds is a difficult conversion, and may require adjustments by your doctor. Do not adjust the dosage on your own or take more than you are currently prescribed without your doctors permission and guidance.
I definitely agree that conversion needs to be doctor supervised!  Everyone reacts differently.
Starting on a relatively low to moderate dose is usual practice- this is incrementally increased depending on Meds .  weight for weight Fentanyl is way stronger- I mean like 100 times stronger , but that's weight for weight . Substitution equations don't relate directly, which is why a decent professional will err on the safe side and increase bit by bit rather than have you OD. The process is called Titrating.

I donk know if its equal, but one thing is for sure with the patch, its a constant delivery..No peaks or valley's, so its constant for the most part. I, myself was never really able to get the same kind of constant relief from an oral medication. Im just saying...I hope you feel better, and good luck
