Strange Symptoms-"COPD" and trouble swallowing?

Here is a timeline of events, although first my history:  

I am 28- I was in the hospital for 4 months when I was born with PPHT and bronchopulmonary dysplasia, I was on oxygen for a year after I got out of the hospital.  I was sick a few times when I was slightly older maybe around 6 or 8 and hospitalized for a week or two with RSV.  I always had asthma when I was a kid and was on Adviar and albuterol.  

For the past many many years (at least 4 or 5) I haven't been taking the asthma meds and haven't needed them.  This Feb I had a gum infection and had antibiotics for that and it cleared up but I still had night sweats and off and on fevers since then.  I probably had 2-3 more courses of antibiotics (as they suspected some infection, or possibly virus) by April because I still felt tired and not right, I was also occasionally having issues with feeling faint/pale/sweating during the day.  

During May or so I started coughing up lots of phlegm (especially in the morning when I woke up) and was having trouble doing so, so I started taking my asthma meds again.  They helped at first.  

July 4th rolls around and I have a strange "can't swallow" sensation, and sort of choked a little at dinner.  This continued for the next week or so and my throat felt like I had a lump or was swollen  I also started to feel like I couldn't breathe but that is was coming from my throat not chest.  Within a week it had seemed to move to my chest, I had 5 days of 50mg prednisone which did nothing.

Since then I've seen a many doctors- here are the tests they have done within the past 3 months, not in order:

Also sorry I forgot to add a little.  I am male.  NEVER smoked, almost never drink.  Am 5'5 180 lbs (trying to lose weight) but I've been that way for years so it's "nothing new."  

A few of the docs are blaming the excessive WOB on anxiety which I'm sure could be some of it because I can't breathe well!  So they are giving me a test run of Ativan (long story short was given a few tabs for blood work-I hate needles- and told my doc it made me feel slightly less annoyed by my WOB)  I used it for the past month .5 MG per evening to relax, today he says try it 3 times a day and see if that helps you at least feel like you are breathing better.  I told him that I want to be comfortable until we figure out what this is, and I know that the anxiety isn't helping.

Here is the rest- it got cut off-

Regular blood tests showed nothing, did a bunch more nuero ones showed nothing- rhumatiod and sed rate slightly high- but no one seemed to think that's the cause/issue
MRI Brain- showed sinus congestion- that's it
Swallow Study in Aug showed nothing
ENT- scoped through the nose, no real findings, I assume he looked at my throat/vocal chords?
PFT- showed Normal FVC  at 80/94%  pre/post meds.  Fev1 was 43/54% which seemed very low
Saw Cardio did EEG EKG all that, and a treadmill test I lasted 2ish minutes and sort of freaked out but said it didn't look like any sort of heart issue.
Switched meds from adviar to Dulera and Spiriva
Chest CT/angiography?  Showed mosaic pattern that didn't change much during inhalation/exhalation and difference between upper lower lobes that they couldn't figure out "what it meant-if anything."
Sleep Study showed I don't have anything "abnormal" no real apnea and no 02 drops, really showed nothing.  I spoke to the sleep pulmo and he also reviewed my CT scan and said he wasn't really sure what/if it meant much at the moment.
I don't know what to do.  I have another PFT for Nov 11, but I'm worried either something else is going on besides crazy severe asthma.  I have to get a blood test for Alpha 1 ( but they don't seem to think it's that, and an IgG? level.)
Any ideas (hopefully ones that aren't tragic) would be appreciated.
