Many Bladder Issues Ruining my life.

My name is James i am 23 years old white male i am 5 ' 11" and weigh around 145 and have been struggling with bladder issues for several years now. It effects every single aspect of my life to the point of depression. I have seen three different Urologists to try and get an answer for this and the best i have been able to get is "Spastic Bladder" which was followed with you are just going to have to learn to live with it. Also been told "You don't know how to pee" That one was really fun. This began around when i was 18

The symptoms i go through every day are: Dull pain (Lower abdomen right where bladder i would guess is) , Difficulty starting urine stream, weak stream, spraying of stream, Frequency (This is a big one). When i eat dinner every night the frequency goes off the charts.

Symptoms i experience often but not everyday. Pain In tip of penis that can't be traces to anyone given area but hurts to touch in a dull sharp pain king of way. Testiculs hurt sometimes i have growths on the veins leading to the testicals but not on them was told this was no need for alarm. Sometimes i do have a burning sensation when i pee but it not every often and its would only be like once that day every couple weeks or so.

Right now i stick to the IC diet very strictly i have been told i do not have IC but its the only thing i have that kind of seems to work since food seems to be such a trigger. I have had many tests done and so far none have even lead me close to an answer. Ive had a Cystoscopy where she found nothing and i was told she seen no she seen no sign of IC or of any scar tissue or tumor ect that could explain the spraying or weak stream. Ive have a digital protate exam where he said it felt a little firm but didnt feel anything for concern. Bladder, ultrasounds, kidney ultra sounds, xrays, Urodynamics test Could only fit 250ml in my bladder before i called it and we got nothing out of it because i could not for the life of me start peeing.

Thanks for posting your query.

I can understand your concern for these symptoms and I agree with you that interstitial cystitis is an important possibility in your case. Other possibility could be eosinophilc cystitis or bladder outlet obstruction( although chances are less because investigations like ultrasound, CT scan and cystoscopy are normal).

Most of your symptoms are pointing towards interstitial cystitis and the diagnosis of this possibility is one of exclusion, as well as a review of clinical symptoms. No known radiographic, ultrasonographic, or other imaging findings are specific for interstitial cystitis and even cystoscopy and biopsy cannot confirm the diagnosis but they are useful in excluding other conditions.

Hence you should discuss it with your urologist and start with the treatment. The general treatment of interstitial cystitis is medications like opioids and tricyclic antidepressants but there are other treatment methods also. These include Bladder hydrodistention, bladder training, medications via bladder instillation like dimethyl sulfoxide (DMS), heparin and Clorpactin and surgery is kept as the last resort.

Avoidance of certain diets like cheese, alcohol, citrus juices, chocolates, coffee, cranberry juice, processed meats, onions, soy products, tea, tofu, tomatoes and yoghurt often help in improving the symptoms.

Diabetes is very big in my family so naturally i have been tested several times, i have had a colonoscopy to make sure nothing is pressing on the bladder nothing was found, Ive been tested for all STD's and STI's all negative (I am sexually active with the same woman for 5 years) I have been told they have found traces of blood in my urine but i have never seen any trace of red in my urine what so ever (trust me i look) nor have i ever had any type of discharge whats so ever. The amount of water i drink every day is small i know i should drink more but drinking the amount i do is the only way i get anything done. it seems when i drink more the frequency comes hard and i pee 3 times what i drank. Even though i dont drink a huge amound i still tend to pee quite a bit every day. Thankfully i work from home because if i didn't i doubt very much i could have a normal job. The medication i have tried is Visicare 5mg once a day (tried twice) and didn't help and Apo OxyButynin 5mg 2-3times a day. didnt work either. in fact these seems to make it worse. So right now i am on nothing other then synthroid for my thyroid. My urologists have just given up it seems and don't really care pretty much telling me i am being a hypochondriac not sure who would make up these symptoms and for what reasons but ive been called one.
One point i feel i should make is when i was 5 years old i was still uncircumcised and got a nasty infection where my penis blew up like a balloon and i had to have emergency surgery. It turned out fine. As for sex that is the one part that seems to work fine. Never have a problem getting an erection or ejaculating no blood ever. I am at a lose in life at this point. it effects everything about me. I can't drink alcohol or eat away from home which has effected friendships. If this is what i got to look forward too for the next 50 - 60 years its not much of a life. Please if you have any ideas or questions post. thank you for your time.
