Life Expectancy of my Father

Namaste,

My name is Russan Dhakal. This is my very first post here. My father was diagnosed of liver cirrhosis 9 years back ( I was at school then).  After recovered enough to continue the normal lifestyle, he seems to have ignored to take proper care of it but only quit alcoholism. Now after all these years, some gallstones were noticed at his gallbladder. While looking into further details, his liver condition also showed up (which I was completely unaware of since he was lately having heart issue and all of us were focused on that issue alone).

He feel severe pain in night due to gallstones but doctors confirmed they can't remove it by operating due to his liver condition. After having a look at the Internet, I came to know about MELD score (his is 12). If anybody could help me understanding the actual condition of my father, I can provide you all the medical details for the purpose.

I'm from Nepal (a small Asian country) and although it's one of the best local doctors we are heading to, he is not counselling enough to meet my queries. If some expert could highlight me with his condition, I deeply appreciate it from bottom of my heart. I already can't thank you enough. I really look forward to hear from you!

i am sorry to hear that your dad is not doing well.  he can take lactulose up to 4 times a day 30cc at a time in order to move his bowels.  He is constipated due to the pain medication..  You can also try stool softener and another good medication for HE is xifaxan which is our best second line agent

Unfortunately, It was known that besides cirrhosis, he had liver cancer and it's already spread to his lymph nodes making it impossible to take any further steps and thus, he's living in morphins, oral chemo, minil and other several medications. Doctor said life expentancy is 6 months max.

I'm writing to you because I'm afraid that he is showing some signs of Stage 2 HE. Just when I thought maybe I should increase his Lactoluse dosages today to 45ml, He didn't even passed stool yesterday night and is developing anxiety feeling too. He is already showing confused behavior.

Sir, Can you please suggest me what medication or whatever actions would be appropriate to get it under control from here?  I have all the reports standing by if you need to refresh some details. I really look forward to hear from you.Thank you!

P.S. There isn't even a single hepatolist where I live and just a single oncologist who already said nothing can be done here. My father is in no stage of travelling to capital again since It's just been 2 days since I got him from another city.

he appears to have cirrhosis from drinking with hopefully some reversibility occurring with ongoing sobriety.  HBV is very common so he should be screened for this as well as having a scan if possible to make sure there is no liver cancer.  this is a very low MELD score (normal is 6) so hopefully transplant will not be needed.
