Incurable rash

My 5 yr old son has had an incurable rash on his bottom for 2 years.  I have tried everything.  3 pediatricians, pediatric dermatologist and an allergist has seen him and were unable to give me an answer on what is causing it.  When I say I have tried everything,  I mean everything. Ruled out strep bacteria and parasites,  no allergies to environmental things from the derm.  I switched to an organic, harsh chemical free soap and shampoo and laundry detergent.  I don't know what else to do.  His rash is raw and painful. He has internal and external hemorrhoids. Constipation, chronic sinusitis and asthma. Getting help from a forum of dermatologists is my last hope to give my son some relief.

Hello and welcome to the forum.  Your poor son.  That sounds like such a difficult experience and situation for him.  We are not dermatologists here, unfortunately.  
You've been working with your doctor and they have not been able to help you, so very sorry about that.  Any update on your son?  Have they ruled out fungal issues? I would guess so with the number of doctors you have visited with your son. Heat rash is something that can happen chronically and happens when the sweat glands under the skin are blocked.  Intertrigo is another condition that can present as a rash on buttocks. https://www.webmd.com/skin-problems-and-treatments/guide/intertrigo-symptoms-causes-treatment-risk_factors_#1 Does his rash ever dissipate? Since you've seen a derm, you've had psoriasis or eczema ruled out? And something that links to skin issues over and over is diabetes. What about milk allergy linked to skin issues in children? Has he had his sugar checked?

I realize I am not much help and want to give you some further ideas to investigate.
