Syphilis from Fellatio

Hey,

Im a 33 old male from Germany. In a relationship since 8 years and married for 5.
Two weeks ago I got oral sex by a female sex worker. I was so drunk I didnt even get an erection. So I stopped her to continue after several minutes. I cannot exactly estimate the time because of the alcohol.

Two days after I got a burning sensation on the tip of my penis. So I visited a doc the next day. He made blood tests, urin tests, and swab tests. All negative.
Yet the burning sensation is still there and becomes even worse. Discomfort on my testicles started, too.
Furthermore I noticed a circular form on my forskin if I retract it. It looks like it is formed by the skin itself. I never noticed it before. It does not hurt.
Now I have big panic that I got syphilis. Acutally I am not sleeping well since the exposure and got several nightmares as well. I just cannot thing straight anymore.

Since all tests were negative, how much likely is it I got an STD, specifically Syphilis because of the circular thing I seem to see.

UPDATE: Today is day 32 after exposure. No physical symptomes appeard by now. I got the test results from day 20. All negative. The nightmare should be over by now. Doc reassured me that Im clean. I will do one last test on week 6 and post my update here.

Thank you for your answer. So here is my update:

My first post was written 14 days after exposure. I didnt mention that I had a full battery of tests on day 10. All came bag negative on day 18. Because of my testicular pain I went to a doc on day 17. I have got an ultrasound screening, urine test and swab test. Everthing was fine, results negative. Seems that it was just anxiety for this problem.

Now to the other problem: Due to your answer I went on day 19 to our local medical center. The doctor said not to worry. Syphilis is nearly extinct in Germany except for homosexual men. And even then it is almost transmitted through anal sex. I should not worry about syphilis and sex with my wife is no problem. Nevertheless  a swab test and blood test was made. Results will come next week.

On day 24 I went to another specialist (urologist). We talked about my worries regarding syphilis and the circular thing. He exemined me and said that is how a healthy penis should look like. Only foreskin wrinkles are forming this shapes. Furthermore he said its nearly impossible to catch syphilis in Germany. He said, I just dont have it and should no think about it anymore.

So today is day 26. My mind calmed down a little bit. As a chancre did not develope by now, how likely it is it ever will?

Typically, the average time for a chancre to develop is 21 days, though it can take up to 90.

However, and I'm going on what your doctors say here, since I'm not familiar with the syphilis rates in Germany, if it's not common there, then the chances of you developing a chancre are slim to none, since the chances of you being exposed are so low.

So I did a little digging. In 2017, there were 7,476 cases of syphilis reported in Germany. https://www.dw.com/en/syphilis-rates-of-the-sexually-transmitted-disease-on-the-rise/a-45331859 The population of Germany is 82.79 million, according to Google.

For comparison, in the US in 2017, 30,644 syphilis cases were reported (https://www.cdc.gov/std/stats17/syphilis.htm) and  1,708,569 chlamydia infections were reported. https://www.cdc.gov/std/stats17/chlamydia.htm

I couldn't find out how many chlamydia infections were reported in Germany, but it's far more common than syphilis, globally.

So I'm not sure if you've been to the same doctor more than once, but you've seen 3 or 4 doctors, and all have said you're fine, and have no evidence of syphilis. Your tests are negative thus far, and syphilis is really rare in Germany. I don't see any reason to worry.

The syphilis incubation period (the time from infection to symptoms) is 10-90 days, with the average of 21 days, so this could be syphilis.

Your test was too early to get an accurate result - it can take up to 90 days to get a positive result, though the average is 6 weeks. Since you have a sore, go back to the doctor and get it checked. You can test positive on a syphilis test as early as a week or two, but they may treat you anyway since you have symptoms.

They may be able to tell you that the sore isn't at all indicative of syphilis, so get it checked.

Your other tests were also done a little early. You can test positive for gonorrhea and NGU in as little as 3 days, but 5 is better. When you go back to the doctor, ask for repeat testing. Since you had oral sex, only gonorrhea and NGU are a concern - you won't get chlamydia and trich from oral. If they run those tests, it's fine. They are all often run together.
