Made a stupid mistake while drunk, now I cant sleep. Please help.

Hi,

I did something stupid while drunk last night. I picked up a street prostitute and had sex with her. Vaginal sex was protected but she performed unprotected oral sex.

One of her hand (not the palm but opposite side of it) had many rashes/sores and I asked her if she did drugs, she said no and that she fell down and got hurt. Now that I think about it, you dont get multiple injuries from falling down, just 1 big one really. Is that a sign of an STI? I read that syphilis create palm rash but this was the opposite side.

I am freaking out and need help. Did she have hiv or syphilis? Can I be catch it? I have some azithromycin. Should I take 2g single dose just in case to avoid syphilis, gonorrea and chlamydia? Is it ok to take it 1 day after contact or should I wait to see if I have symptoms? I understand I shouldn't be taking antibiotics without prescription but I do have them so can I?

Also, I remember she had what looked like hemmroids or warts on anus, although I didnt have anal sex. I have never done anything like this and have been in a long term monogamous relationship and I am so disgusted with myself right now and feel like jumping off a cliff.

Please let me know if azithromycin 2g will fix any potential disease.

I don't know - I fell down the stairs once, sprained a wrist and broke a bone in my foot. It's possible to have more than one injury on a fall, and the injuries were just to the back of one hand, so maybe she fell on gravel or something?

In any case, that doesn't sound like any STD.

So receiving unprotected oral sex puts you at risk for syphilis, genital herpes type 1, gonorrhea and NGU.

Syphilis isn't that common, and she'd have had to have a sore in her mouth to transmit it. You wouldn't see symptoms of this for 10-90 days, but the average is 21 days, and you'd get a sore called a chancre. This doesn't cause burning, pain, discharge, etc. You can test for this at 6 weeks. If you get symptoms, but test negative at 6 weeks, test again at 90 days.

If you don't already have herpes type 1 (think oral sores, like cold sores but not canker sores), then you could get genital herpes type 1 from receiving oral sex. This can happen even if the person performing oral doesn't have a sore, but it's more likely if they do. The time from infection to symptoms is usually 2-12 days, but the average is 4 days. You can test for this now, and then again at 4 months to make sure you don't have it. If you test positive now, it's a pre-existing infection that you had before you were with her.

You'd see symptoms of gonorrhea (if you get symptoms) at about 2-5 days, and this would usually be a discharge, burning, etc. Some people don't get symptoms. You can test for this as early as 3 days, but 5 days is better. You can have a urine test or a swab test.

NGU is an infection in the urethra that is caused by anything other than gonorrhea (nongonococcal urethritis urethritis, sometimes called NSU, for non-specific). This can be caused by normal mouth bacteria entering the urethra, and the symptoms and testing times are the same as gonorrhea.

There is definitely a risk here, but oral sex is lower risk than intercourse.

Remember that she uses condoms to protect herself, not you. Her interest in remaining disease free is as important, if not more so, than yours is.

Alright man here's the deal.
First and foremost if you're STILL in this "monogamous" relationship, it's time to own up. Tell the truth. You owe your partner that much.

In regards to everything else going on with you,

The rollercoaster jjjjjuuuuussssssttttt passed the top and the ride has just started and you're now screaming that you want to get off.

Tought ****.

I say this from a place of love. I've never slept with a prostitute but I put myself through STD roulette a lot. This metaphor (being on a rollercoaster) has helped me with my std anxiety attacks. Gotta accept your choice and just live a life of abstinence for the next 30 to 40 days. Once that marker hits, go and get an STD test. If it comes back negative there's a good chance God let you dodge a bullet. (If you're a God Points person, telling your partner the truth might help you. In case you need selfish motivating factors for doing the right thing. We're all human!)

Obviously if anything shows up between now and then, you'll have a better idea of your situation. If symptoms show up, go to a doctor sooner..

Don't take the antibiotics right now. Bad Idea.

Everytime you have an anxiety attack about this stuff try to picture yourself on a roller coaster ride. Your choice to ride the ride has been made and you are now powerless to how the ride plays out. The "ride" ends in 40 days or when REAL actual symptoms show up.

Until then you are stuck on the ride so don't waste your time or energy trying to get off of it early (panic and anixety attacks)

Thank you for your response. I will think of it as a rollercoaster.

Still I haven't slept the last 2 nights. The guilt is probably worse than the fear. I don't know how to tell my partner. I will try but I am not sure I can. I will abstain until I get tested and hope for the best.

I know this comes out as me being a bad person, but I can't hurt my partner. I will never do anything like this again and the overwhelming shame and guilt is already too much to deal with. Maybe I will come clean after I get tested.

To the doctors reading this: I read that getting HIV from blowjob is almost impossible. But the rashes/sores on her hand can be a viral load as per my reading. Does that increase my risk of getting HIV from unprotected oral and protected vaginal sex? I can get treatment for everything else if it comes to that or atleast get tested in a few weeks to be sure but this is too scary. Also, how soon can I test for HIV and syphilis? I have read conflicting information on some sites. Thanks in advance for your help.
