Question about fertility

I have been trying with wife to conceive I did semen analysis and receive these results any help on improving and getting pregnant

Exercise and sleep, Reduce stress, stop smoking, avoid excessive alcohol use or drugs, avoid certain medications (article has a list), Fenugreek supplement, Vitamin D and calcium, Indian ginsing, foods high in vitamin C, monitor your fat in foods, eat healthy fat and minimize unhealthy fatty foods, avoid exposure to environment toxins or occupational hazards, avoid soy and get enough zinc and folate.  In other words, you can take a good multi vitamin and eat a healthy diet.  

My doctor also told my  husband to wear boxer shorts rather than anything tight and to avoid hot tubs or things that increased the heat of that area.

Hi.  I wish doctors would go over results WHEN they give them to people.  I am not an expert in this area and didn't know they actually ran so many numbers on sperm!  Is your wife also having testing done to see if there are issues with fertility for her?  Here is a really good article on sperm count and fertility.  https://www.mayoclinic.org/diseases-conditions/low-sperm-count/symptoms-causes/syc-20374585  Your quantity looks to be in the normal range. I think.  It's hard to tell from the way these results are written, however, I know you wrote them like you have them and they actually tried to make it easy to read they thought.  Basically, You are considered to have a low sperm count if you have fewer than 15 million sperm per milliliter or less than 39 million sperm total per ejaculate.

When are you seeing the doctor to discuss this?  There are lifestyle things you can do to help with sperm count.  Want me to list some?
